Character mistake: Francine's phone fling storyline says that she's a spy that asks the Player's help to send encrypted files disguised as photos, and they have to pretend to be lovers trading pleasantries and pictures next time she calls him. She says that she's going to turn off the jammer (which allows her momentarily to talk without being intercepted) "now" and says goodbye. When hours later she calls, she begins the conversation (and the act) with "Ready? Here we go." So she gives herself away.

Character mistake: In one of her lines reacting to the Yandere character, Nina says that she took the batteries out of the alarm in "the servant's wing." It's the servants' wing, plural (there's a literal army of maids there).

Continuity mistake: When the cops tear down the board with the serial killer case, you can see already next to the lawyer the note in grey ink that says that the object used to suffocate him was wrong, which is a conclusion Bishop comes to later in the case resolution, with the notes on the other two victims in the same ink. Continuity mistake: A woman approaches Oscar Finlay when he's having a beer at the bar. She asks him "What do you do?" and his hand is at the bottom of the glass, only to be at the top of the glass in the reverse shot. It happens again as the two different angles alternate. (00:36:05)

Stupidity: To make the hit, The Killer places a bomb on the ceiling of his room - which is the floor of the victim's. He has absolutely no way to know where in the room the man will stand and when we see the explosion, it does not even touch the couch. The brilliant plan of the flawless killer is anything but brilliant or flawless.

Continuity mistake: When Jim picks up the box with the mission briefing, his hand in the close-up of the fisherman's case is at the bottom of the box, but it's on the opposite side when we see Peter Graves retrieve it. (00:07:00)

Bug: When you persuade an officer in a debate, one of the possible lines for the outcome is "You have shown me where I stray", but it is your character who says the line, not the person you just brainwashed into being hired.
