He puts the coffee on the table, and in the next shot you can see already on the table the file that he never put down and is in fact still in his possession in the shot that follows. (01:21:05)

Continuity mistake: Dr. Werner Ernst is talking with nurse Stella as he has his cup of hot liquid. She asks him "Is the world really that shallow?" James Spader is holding the cup from the top without using the handle, but in the next shot he is holding it from the side instead as he puts it down on the desk. (00:04:35)

Other mistake: The "Young Man's Diary" the protagonist reads in the prison cell contains a major inconsistency - it is dated October 10th, saying the escape will happen at 11 PM, but other diary entries such as Janitor Andy's and Commander Vincent's contradict it saying the escape happened "the night before" on their October 10th entry. It also has a strange name ("Fellipe", quite unlikely spelling of his prison mate's name) and a typo ("Unit B will use a rope climb down", missing "to" or "and").

Continuity mistake: Belson releases the mooring line to take off with the boat, and we see the rope slip away in close-up. Too bad that in the shot that follows the ignition, the boat is still moored, or at least the line is still in place. (00:55:35)

Continuity mistake: The first time Mrs. Ingelthorp is shown, she is holding a pen. In the second shot she is holding it in her right hand, while in the third she has the lid of the pen in the right hand and the pen itself in the left. (00:07:30)

Other mistake: Freeze's hideout is in an abandoned oil rig, with its tanks full to the brim of oil. You'd think the company owning it would have drained the millions of dollars' worth of content first, before 'abandoning' it.

Continuity mistake: After beating the two robbers up, Batman and Robin dump them in a pile of trash on the sidewalk. That pile of trash was not there when the car turned the corner and crashed in front of the grocery store. (00:10:35)
