Continuity mistake: After they load the woman from the wrecked car into the ambulance, Roy and an ambulance attendant are riding in the back with her. In the shots of the moving ambulance, until it's involved in the traffic accident, we should be able to see Roy and the attendant through the windows, but they're simply not there.

Continuity mistake: When they bring the Hermit into the ER, Brackett tells them to take him to treatment 4 and he's wheeled there, but in the shot from inside the treatment room we see that he's actually inside treatment 1.

Continuity mistake: Before Old Bill wakes up, Brackett, Morton, and Early are talking in the hallway outside Bill's room, and the hallway is a big, wide open space. After Bill wakes up, Dixie goes out to the hallway to get Brackett, and when they walk into Bill's room we can see the hallway behind them, and it's a fraction of the size it was before, only about 4-5 feet wide now.

Revealing mistake: When the critical boy who ingested the ant poison is being wheeled into the ER's treatment 3, we can see Johnny laughing, and obviously since absolutely nothing about the situation is funny, we know it's Randolph Mantooth just cutting up during the take.

Continuity mistake: After bringing Old Bill to the ER, when Squad 51 gets back to the station, in the shot of the squad driving backwards toward the driveway, the engine we see parked in the apparatus bay is the Crown from the first two seasons, not their current Ward LaFrance.

Continuity mistake: When Station 51 is dispatched to 'man down', Cindy and her mother are standing in front of the rear bay door, but they vanish in the next shot as the guys get ready to leave.

Revealing mistake: When Cindy and her mother come to see the station, Cindy goes to the closet with the gear, and as she opens the door Cindy starts laughing before Johnny even gets hit by the phantom, and then she glances toward the camera grinning because of the prank.

Visible crew/equipment: After Station 51 responds to the unknown type rescue, Engine 51 leaves on another run, but when Johnny and Roy go into the house with the wife of the man who lost his voice temporarily due to the dieffenbachia plant, the bulging outline of the lavalier mic transmitter and wire are visible on the back of her white robe.

Other mistake: After Johnny gets hit by the phantom in the apparatus bay, Station 51 is dispatched to an unknown type rescue at 617-1/2 Kimball Rd, and before the dispatcher even gives the address, Mike has already located the area on the map with his finger.

Other mistake: While John is searching through the papers for the receipt, when Roy takes the milk carton out of the fridge he pours some into a glass and some ends up spilling onto the floor, we can see there's black tape on the milk carton covering the brand name.

Continuity mistake: When Brackett's examining the teenager (Donny Most) with paralysis Morton's standing beside him, but in the next shot as Dixie takes the teen's BP, we see Morton standing behind the teen with the student nurse, then Morton's back beside Brackett.

Continuity mistake: When Dr. Early and Marcia's father stand outside her semi-private room, number 104, we see treatment 4 is right beside it. There are two problems with this bizarre situation. First, when Dixie gave Early the lab results about Marcia's possible pregnancy earlier, treatment 2 is right beside treatment 4, not the semi-private room 104. Second problem is that when Early and the parents walk into Marcia's room, in the interior shots we can see that there's a perpendicular wall right beside the doorway to its left, where the hallway ends. Visible crew/equipment: While the guys are at the table eating Marco's stew, when Johnny remembers that he spent $8.42, numerous set lights are reflected on the coffee pot's chrome surface.

Revealing mistake: When the guys are in the backyard with the woman who fell through the ground into an old abandoned well, in the overhead shot we can see the cuts that created the cutout hole on the flat surface around her, and that it's not really dirt or sand that she's slipping through.

Audio problem: After Johnny gets out of the elevator, Johnny sees a crowd around the treatment room, so Roy tells him that they're interviewing the bank guard who was shot, and when we hear Roy ask, "How'd you make out with the girl from orthopedics," his mouth is saying something different.

Continuity mistake: When the guys are eating dinner talking about marriage, John's glass of milk is half full in the shot from behind him, but when Barbara calls the station and Captain Stanley answers the telephone, John's glass is nearly full in the shot facing him.

Visible crew/equipment: At the horse ranch, after the man falls off the horse John and Roy rush over, and as Johnny leans over him the bulging outline of the lav mic transmitter is visible on the back of Johnny's jacket.

Character mistake: When Johnny and Roy respond to the 'possible injury at the horse ranch', after the guy who was knocked unconscious gets back on the horse, Johnny tells Roy, "Why don't we stick around for another few minutes, unless we get another call." They did not contact the dispatcher that the squad's available, so they would not even be dispatched to another call, since they're already on that run.

Continuity mistake: After bringing in the atropine overdoses from the daffodil bulbs, in the shot from inside the treatment room when Roy and John leave treatment 3, which is opposite the nurses' station, they turn left to head towards receiving, but next shot they're walking from around the corner in the hallway, then pass that door they just left from.

Audio problem: After John starts telling Roy about how he and Barbara went to a movie, when Squad 51 is dispatched to 'man unconscious at Observation Park', we see a closeup of Roy in the squad as he puts the gearshift into drive, and then we hear the audio of the ignition being turned on.
