Visible crew/equipment: S4xE8 Meet Kevin Johnson: Near the end, Karl has nothing in his left ear when he and Alex sit down and start talking, but when Karl falls onto the ground, the actor has a wireless earwig device in his left ear in his close-up, before Alex turns him over. Then, when Alex sits by the tree distraught, Danielle tries to convince her to listen, and in Alex's extreme close-ups, she has a mini lav mic (similar style to DK3 series) at her right ear that becomes visible.

Continuity mistake: Near the end, when Sir Mador is hit with the small crossbow bolt, the impalement changes. In the first shot it's protruding from his abdomen well above his belt, but in the next shot the crossbow bolt is now lower, actually pierced through Sir Mador's leather belt. (02:00:30)

Later, while Phoebe sits on the desk during detention, her belt buckle is missing in her close-ups, and it reappears mid-shot as she crosses her legs. Then, when Phoebe's classmates share how they miss the "old" Phoebe, her buckle and the two brown pouches vanish, but they all reappear in a moment. (00:05:05)

Continuity mistake: As Raoul is being chased by the highway cop, the camera switches views between the rear view mirror and looking through the back window. The line in the middle of the road changes unevenly between passing and no passing lanes.

Continuity mistake: When Gonzo is trying to kill himself in the bathroom, there is a watch behind him that disappears and reappears.

Visible crew/equipment: Watch the scene where Frank gets killed. In the last shot of this scene, we see a soldier standing over the dead body. Keep an eye on the right side of the screen as the camera slowly moves away and you should see one of the cameras and a cameraman on the ground next to a large metal object.

Visible crew/equipment: While Myra is lying on the table before her surgery Dr. Mike tells her she's almost ready to begin, then just as Horace is about to stand up the orange T-mark is visible on the floor. (00:38:30)

Visible crew/equipment: When Stevie gives Sonny the handgun, as Sonny tucks the handgun into his waistband, the actor's mic wire becomes visible. After Moretti's first phone call, Sonny is angry that "Jack downtown" gave him wrong information, and when he stands up there's an odd bulge at the back of his pants, presumably his mic transmitter. Then Sonny asks Mulvaney about the back door's location, and when Sonny leans down to tell Sal, "Cover them, cover them," we can see the mic and its wire between Al Pacino's tie and shirt. In exterior shots, we can see the outline of the white tape under Pacino's shirt (holds his lav mic in place due to the actor's frenetic movements). (00:07:55 - 00:20:40)

Continuity mistake: After the fight ends between Dalton and Jimmy, Jimmy is killed and kicked into the water. When Doc pulls Jimmy's body out of the water and onto the sand, Jimmy's eyes are wide open and his mouth is open. Immediately after, in a different camera angle, Jimmy's mouth is now closed and his eyes are shut before Doc even touches his eyes.

Visible crew/equipment: While Moretti is in the barber shop on the phone with Sonny, the camera tracks closer to Moretti, Sheldon, and Carmine, and as Moretti says, "Come on, let's be reasonable...just give me your name," we can see their three T-marks (dark chalk) on the floor at their feet. Other mistake: While Ann Darrow is lying on the Empire State Building ledge beside the ladder, the backdrop shows the Chrysler Building, Queensboro Bridge, and Roosevelt Island, behind to the right. However, after Kong falls from the skyscraper, when Jack appears and says, "Ann, hang on, dear," this one shot is angled toward the left of the ESB, but the set's backdrop is positioned so it's the same background with the Chrysler Building, Queensboro Bridge, and Roosevelt Island, behind to the left. Continuity mistake: When King Kong carries Ann Darrow to the top of the Empire State Building, he places her on the ledge near the fixed ladder mounted to the building. During the scene, in Ann's close-ups, the ladder vanishes from where it should be, and in the zoom-in shots (biplane POV), the ladder is mounted in a different area than in the long shots. Revealing mistake: While King Kong is atop the Empire State Building, the biplanes are strafing their large target. There are several static close-up shots facing a biplane's nose, and in these "aerial" shots, there's a girder beneath the set prop's fuselage. (01:36:10)
