Continuity mistake: While Don's at the parking garage with Rachel Abbott's car he realises that the killer had been watching Karen Silber's house, and when they rush off to her house there are white security bars on the windows and front door in the long shot and also later in the news footage, but when they're at her house none of those bars exist and the shrubbery also differs.

Continuity mistake: When the paramedic examines Foggy he places the stethoscope around his neck with the chest piece hanging to his left, but when it cuts to the view from inside the ambulance the stethoscope's suddenly hanging around his neck the opposite way. (00:17:30)

Continuity mistake: At the Resistance base, when the doctor's treating Chewie's injury, she's using a medical device with her right hand as she's telling him that he must be so brave, but when it cuts to the next shot it's her left hand up using the device. (01:22:55)

Continuity mistake: During Nadya's escape from the monastery, there are exterior shots of Nadya through the windshield and there's nothing hanging from the rear-view mirror, but in the interior shot (viewed from the back seat) just as she swerves away from the red car, there's a small cross on a cord which is hanging from the rear-view mirror.

Continuity mistake: When Eddie and Joann leave Satin Records they get in the car and there's a rear-view mirror on the windshield, but it vanishes while they're driving, and then it reappears.

Visible crew/equipment: After Kirk's briefing regarding Mitchell's abilities, when Spock and Kirk are alone just as Spock says that they'll never reach an earth base with Mitchell aboard, the boom mic casts a shadow overhead until Spock steps forward, and a bit can be seen again when Spock tells Kirk that it's his only choice. Continuity mistake: When the Thasians restore Yeoman Rand on the bridge she's barefoot, but when Charlie begs Kirk not to let the Thasians take him because he can't live with them anymore, we see Rand wearing a white fluffy slipper (which she wasn't holding when she reappeared), then she's back to being barefoot again. Visible crew/equipment: After Uhura tells Kirk that "Captain Ramart of the Antares is on D channel," Kirk and Charlie head to the bridge, and when it cuts to Uhura the actor's tape mark can be seen on the floor behind her, where Charlie will stand when he steps out of the turbolift.

Visible crew/equipment: When Spock and McCoy walk into Kirk's quarters, in the wideshot the white tape mark can be seen on the floor under the desk at Spock's feet, before Spock requests permission to beam down to the planet, and can be seen again when Kirk stands up.

Continuity mistake: When Uhura tells Kirk that she's made contact with his private transmitter she's seated at the communications console, with the two overhead screens above her station, beside the turbo lift alcove. Yet after the contact's been broken, when she tells Kirk, "It's impossible at the moment because they stopped broadcasting," in this unusual shot facing Uhura the turbo lift alcove is not behind her - Uhura is actually seated much closer to Spock's station, note the upper section with overhead screens. Then she's properly seated at her station once again.

Factual error: This episode takes place in NYC during the 1930's, but after McCoy's phaser is stolen in the exterior shot of the buildings a fallout shelter sign can be seen. These signs were put up no earlier than 1961.

Continuity mistake: When McCoy arrives in NYC while the man is stealing a bottle of milk, in the closeup the actor's ring finger is partially amputated, but when he steals McCoy's phaser his finger is intact in the closeup.

Visible crew/equipment: After the Klingon vessel's attack on the Enterprise, while returning fire just as Kirk orders to maintain firing range at 100% dispersal pattern, the set's unfinished wood plank with grey paint drip marks can be seen at the bottom of the screen. Additionally, at the end when Spock goes to stand beside Kirk's chair (also when he walks away) the actor's mark can be seen on the floor by Spock's feet.

Continuity mistake: After Rachel spins the bottle and it points to Joshua, when she crawls over to him on her hands and knees the bottle's in front of her knees, but in the next shot facing Joshua the bottle is to her side, near her feet.

Continuity mistake: When Nadya escapes with her son from the monastery she drives the car through the doors, and the driver's side of the car sustains substantial damage at the side of the hood with the side-view mirror completely broken off, but during the chase there's little damage to the car's body and the driver's side-view mirror is now dangling at the side.

Revealing mistake: At the start, just as Moreau rides up the stone road leading to the monastery up ahead, before he passes the two guards we can see that it's a flipped shot - note the backwards license plate number.
