Revealing mistake: After the two drivers from the traffic accident walk off holding hands, and Johnny tells Roy what happened with Barbara, while Roy explains "body-language" to Johnny, we can see the white tape with "Kevin" inside Roy's helmet.

Revealing mistake: At the apartment complex where the jet fighter had crashed there's an explosion, and when one of the firefighters becomes engulfed in flames his helmet falls off, and the special fireproof headpiece that the stuntman is wearing is visible.

Revealing mistake: At the start of the episode, we see Squad 51 speeding toward their destination, and when the camera cuts to the woman in front of the market, she's waving her arms to the squad somewhere in the distance. But notice that while Rosa is looking to her left and waving, we can see Squad 51's reflection in the window (with the liquor bottles), as they are parked right there, just off-screen waiting for their cue to "speedily" pull up.

Revealing mistake: When Squad 51 leaves Rampart, in the first interior shot facing Roy as John says, "Yeah, I'll tell ya, when it hits ya. Boy when I met Valerie it hit me," through the back window we see movement at the back of the squad. Then in the shot facing Roy as John says, "So that's that... That's exactly how it is," through the back window we see a crew member's face (sideways), he has a mustache (John does not have a mustache), right behind John's head, as he's riding atop the back of the moving squad. Revealing mistake: When Captain Stanley runs into the burning house looking for Johnny, the floor beside the stairs (under the table) is visible behind him, and there is no rug only wall-to-wall carpet, but when Johnny and Toby fall from the stairs, that area of floor is covered in a rug raised up like a platform, covering the crash mat for the stunt, then the rug vanishes when the guys get Johnny and Toby.

Revealing mistake: After Station 51 is dispatched to "traffic accident with injuries," when they're on scene at the plant with the injured tanker driver, pay particular attention to the background behind Engine 51 while Roy is on the Biophone with Rampart. When Brackett says, "51, can you give me any more information," we can actually see Station 51 aka 127, directly behind Roy.

Revealing mistake: While 51's on scene at the traffic accident involving a car that broadsided an ambulance, when Chet opens the back door of the overturned ambulance the license plate reads "99923U" but the "U" is lopsided and pushed to the edge of the plate, which makes it obvious that it's a magnetized letter. It's positioned properly in following shots.

Revealing mistake: After Johnny's line gets snarled while testing Wally's equipment at the studio, when Roy lowers himself on his own line wearing his lifebelt, we can see the added carabiners that are attached to the lifebelt which go down into Roy's pants, and are presumably attached to the extra support the actor is wearing under his pants.

Firehouse Quintet - S6-E18

Revealing mistake: At World Picture Corp, when John and the guy with the broken clavicle are lowered from the overhead joists they're both wearing lifebelts buckled around their waists. The problem is that once they reach the ground, we can see the additional carabiner attached to John's lifebelt, which is also attached to extra green support protruding from under John's shirt. We can also see the victim's additional carabiner going under his flannel shirt. While Roy is on the line with Rampart, when John removes the victim's lifebelt, the extra carabiner is still protruding from under the victim's shirt and John places his hand over it. (00:27:00)

Revealing mistake: After Johnny joins Roy in the ambulance and the patient flatlines they continue to Rampart, and when Brackett asks 51 for a strip, if we look out the window that's beside Johnny, we can see GlamourTrams in the distance. And when Roy says, "Isopeterenol drip, 10-4," we can see GlamourTrams right outside the window. They are driving around Universal's lot while shooting the footage inside the ambulance.

Revealing mistake: After the MI patient from the construction site is brought to treatment 1, when Early uses the laryngoscope for an endotracheal intubation, we can see that he merely places the laryngoscope at the side of the man's face, especially since we can still see the entire distal end beside the man's neck.

Revealing mistake: When John and Roy reach the man lying near the pool with a back injury, while Thadeus keeps gnawing and grabbing at Roy's pant leg, we see the extra material that Roy has under the pants (over his socks) to prevent the dog from accidentally harming his legs.

Camera Bug - S4-E10

Revealing mistake: When they wheel the stretcher with the unconscious teenager from the school fire into treatment 3, the stretcher is pushed up against the ER's gurney, and when the teen's right arm gets caught between the stretcher and gurney, the supposedly unconscious teen moves his arm up out of the way. Then he lifts his left hand when the attendant removes the blanket.

Revealing mistake: After rescuing the burn victim from The Boat's explosion, Johnny and Roy are at the back of the camper with the victim, while the victim's wife drives the camper to the clinic, but when they arrive at the clinic the camper hasn't even stopped yet, and we can see the woman's legs at the side of the camper, waiting for her cue to come rushing to the camper's rear door when her husband is brought out.

Revealing mistake: While the guys are eating chilli at the table, when Station 51 is dispatched to 'man down', notice that when they're at the scene, both paramedics are wearing their jackets and Johnny is wearing his helmet. We never see Johnny's face in any of the wide shots, because the entire time it's the double with Roy. We only see Johnny alone in a few closeups, pickup shots which were filmed later, and we can see the style of the houses in the closeups do not resemble the houses in the other shots. The real Johnny is back, once they reach the hospital.

Firehouse Quintet - S6-E18

Revealing mistake: When Station 51 is dispatched to the gas explosion at the cafeteria, during the actual rescue the four firemen we see in closeup in the cafeteria are supposed to be Roy, Johnny, Chet, and Marco, but it's really odd that all are the doubles, very obvious in the closeups. There's little dialogue during this scene, which was dubbed later in ADR.

Upward and Onward - S6-E22

Revealing mistake: At the movie studio, when the actor who plays Dr. Ned is loaded onto the ambulance stretcher, their set's 'fake' window area is the same as Rampart's 'real' hallway later in the episode. We can see this when Brackett, Early and Dix walk down the hallway to kick Dr. Ned's film crew out of his room. Pretty funny to see the same chairs, wall, window with painted exterior backdrop and color stripes on the floor. This area of Rampart can also be seen in the previous episode 'Family Ties', when the clock radio shorts out causing a fire in the patient's room.

Revealing mistake: When Station 51 is dispatched to 'fire in the salvage yard', in the exterior shot as the squad and engine roll out of the bay, we can see the real Station 127's vehicle is still parked inside the apparatus bay, near the rear bay door.

The Indirect Method - S5-E6

Factual error: When Roy is electrocuted and falls from the roof, after Karen uses the defibrillator paddles on Roy, she lifts both paddles, looks at the EKG monitor and says "He's converted." How exactly could Karen have known that he's converted? It's impossible for the EKG monitor to show anything at all. Either the defib paddles have to be in contact with Roy's body for the "quick-look" to get a reading, which they weren't, or the ECG electrode discs have to be on Roy's chest connecting him to the EKG monitor, and they weren't. As an aside, just watching Marco having problems attaching the air mask, and quickly glancing up towards the camera frustrated, then giving up is priceless.

Roy: Now, wait a minute. We're not playing games. We don't play games with the equipment...
Captain Stanley: Wait, hold on. C'mon everybody, look we're all on the same side here, aren't we?
Charlie: I wonder.
Captain Stanley: Charlie, why can't we put the squad in the shop and have us a reserve vehicle here?
Charlie: No, no, Hank. Not until I'm 100% sure that I can't fix it. If there's anything wrong with it. This coffee stinks. [Leaves the room.]
Captain Stanley: You guys playing some kind of joke on him?
John: We didn't make the coffee!
Captain Stanley: Not the coffee, you twit, the squad.
Roy: Cap, there's something wrong with that squad out there.
Captain Stanley: [sigh.] All right, if you say so... I made the coffee.

School Days - S2-E14

Question: Who are the two baseball players that walk up to the nurses' station to talk about their teammate and discuss his relationship status with his girlfriend to Dixie?

Answer: I was rewatching a few first season episodes of Charlie's Angels (1976), and in S1xE6 "The Killing Kind," I recognized the same actor. So, to finally fully answer your question, the two baseball players in School Days are played by Rod Perry and Sean Fallon Walsh.
