Character mistake: After Captain Stanley tells the guys about the diet and nutrition study, Cap tells Roy that he and John should go to Oceanside Park for a fire inspection, but later when Johnny's on the HT he tells dispatch to respond Engine 51 to Oceanland Park, not Oceanside Park, and then the dispatcher gives the incident address as Oceanland Park.
Character mistake: When Brackett and Early are dealing with three squads at the base station at the same time, Squad 51 has two victims - father and son, Squad 18 has a pulmonary edema, and Squad 36 has only one bicycle rider with severe head injuries, because the second rider died, so the total is four out of five being dealt with. Back at the station, after Johnny asked Dixie about the condition of all the victims, he tells Roy that the father and son are both responding, so is Squad 18's pulmonary edema, and for Squad 36's bike accident, that "both" are coming out of the coma, which totals five victims. Then Roy responds, "Four saves out of five victims. That's pretty good." Johnny should not have said, "both" since it was just one rider.
Character mistake: Days after Drew's death (Roy and Johnny had two days off after Drew's death), during the meeting that Brackett has with the four paramedic units, one of 59's guys asks, "What do we do when we have this overload situation on the same frequency? Two squads, like 51 and us today?" The situation with the frequency did not happen "today" as he said, it happened at least three days prior.
Character mistake: In the morning, when Roy and Johnny are by the lockers, talking about what Johnny did on his days off, Roy tells Johnny, "I came on this morning, guys on A shift told me that they had a meeting with Brackett at Rampart yesterday." It could not have been "A shift" because Roy and Johnny work the squad for A shift, with Captain Stanley and the rest of the guys. And we know that Roy and Johnny didn't switch shifts with B or C, since the rest of A shift are there with them, working the engine that day.
How Green Was My Thumb? - S3-E17
Character mistake: When Johnny is filling the radiator on the squad, he pays close attention to not over fill. After Chet comes in and asks a question to Roy, the alarm goes off, Johnny closes the squad hood, without pulling the radiator cap on.
Character mistake: After Squad 51 joins Engine 51 at the heart attack incident, while Roy's on the Biophone with Rampart he sends a second EKG strip, and Early orders, "100 milligrams lidocaine bolus," but he mispronounces lidocaine and says, "Lidograin," then he pronounces "Lidocaine drip" properly.
Character mistake: When Johnny and Roy respond to the 'possible injury at the horse ranch', after the guy who was knocked unconscious gets back on the horse, Johnny tells Roy, "Why don't we stick around for another few minutes, unless we get another call." They did not contact the dispatcher that the squad's available, so they would not even be dispatched to another call, since they're already on that run.
Character mistake: When Johnny is about to tell Roy what the problem is, the dispatcher drops tones and says "Station 51, Engine 36, Truck 116, small aircraft in trouble.," but when Captain Stanley responds we hear him say, "Squad 51, 10-4...," even though he should have said, "Station 51, 10-4...," because both Engine 51 and Squad 51 were dispatched.

Character mistake: When Johnny and Roy are in the kitchen and start reminiscing about when they first met, Johnny recalls those days and says "I was working B shift at Station 8's," however, Johnny was actually working at Station 10.

Character mistake: When Dixie tells Gail to set up in treatment 1, Brackett tells them they can't use treatment 1 because they have a skull fracture coming in, then Brackett and Early head down the hallway to treatment 1, and that skull fracture, Skyle Tyler, has already been lying there being examined by Morton.
Character mistake: After the cat has her kittens, when Squad 51 is dispatched to the 'man trapped in boating accident', we hear Captain Stanley respond, "Station 51, KMG365," even though his response should have been, "Squad 51, KMG365," since it's only John and Roy that were dispatched, not the entire station.
The Tycoons - S5-E23
Character mistake: Chet states that he stops for breakfast at the hot dog stand. When Roy and John are eating there, a sign in the background says it opens at 11:00.
Captain Hook - S6-E8
Character mistake: When Johnny and Roy are with the heatstroke victim in the polar bear costume, they're on different sides of the victim when Roy says, "Twitching here in both arms," and Johnny looks at her left arm and responds, "Yeah, the same thing over here too." And then it happens again, when Roy says, "Got some heavy spasms in these arms," and Johnny responds, "Yeah, same thing over here."
Character mistake: At the construction site, while the paramedics are defibbing the cardiac arrest Stoney's on the line with Rampart, and Brackett orders, "Administer 5ccs 1 to 10,000 epinephrine IV," to which Stoney responds, "Administer 5ccs epinephrine, 10-4, LA," even though he's speaking to Rampart not to LA dispatch.
It's How You Play the Game - S4-E19
Character mistake: On scene at the vehicle accident, John palpates a pulse on the driver's leg and tells Roy that he's got a good strong foot pulse. Then when Roy's on the line with Rampart we hear him say, "He has a strong fetal pulse," but presumably Roy certainly must have meant to say, "strong pedal pulse."
Character mistake: When Mr. Grady, the self defense instructor, and his wife are in the ER waiting room after Brackett taped up his fractured ribs, Mr. Grady makes a comment about being flipped, and instead of saying Mrs. Pestone he says the name incorrectly as Prestin, then a few moments later he says her name properly, just as he did earlier during the class.
Character mistake: At the restaurant, when Captain Stanley's on the Biophone with Rampart, Brackett asks, "Is there any evidence of external hemorrhaging on your second victim?" and then Cap responds, "We can only see superficial cuts at this time... Pupils are equal and reactive, negative on the IV at this time," as if Brackett ordered an IV, but Brackett never said anything to Squad 51 about any IV.
Character mistake: When the guy who was injured in the manhole is brought to treatment 6, he's transferred onto the ER's gurney and we see Carol taking his pulse, but then she tells Early, "Pulse 120, respiration 24," even though she never checked his respiration rate.
Fair Fight - S6-E5
Character mistake: When Squad 51 is dispatched to the fighting couple the first time timeout is 10:12, and the second call to that house timeout is 12:37. When Roy and John are there the first time, it's for no more than five minutes. Then when they're there the second time, Roy says "Please folks, we just went through this less than an hour ago," though it was over two hours when they left the fighting couple the first time.
Heavyweight - S3-E5
Character mistake: When Johnny first contacts Rampart via landline about the stabbing victim, while Roy is across the street with the gunshot victim, Brackett gets on the line and asks Johnny if he started an IV, to which Johnny replies no, then he asks what the vitals are. Johnny has only just made contact with Brackett about this patient, so Johnny could not have started an IV without the doctor's consent, Brackett knows this. This goes against established procedure.
