Continuity mistake: Doug En Vogue: While Doug is at the store, he tries to attain a new original look for himself. When he's looking at his reflection dressed as a rapper, the necklace with the word "RAP" is not a reversed image, but a moment later Doug's reflection is properly backwards as it should be. Visible crew/equipment: After Paris hails Tuvok about the problem in the mess hall, a brawl breaks out. When the new Hirogen Alpha enters the galley to access the com relay, just as this Alpha removes the front panel in order to transmit a signal, the actor's green tape mark is visible on the floor. (00:39:00)
Continuity mistake: At the start, when the carful of teens pulls up alongside the convoy's lead vehicle, in the first shot of that lead driver (his reflection is in the round wing mirror), the side of the hood reads "U.S. Army" and "276***," the partial serial number beneath. Then, in the next shot and subsequent shots, the lead vehicle's serial number reads "1B7731" to the side of the hood. Just to add, when Spalko arrives at Hangar 51, her vehicle has that "276" serial number with license plate W-539. (00:01:05)
Continuity mistake: At the start, just as the carful of teenagers joins the convoy of military vehicles on the road, there are very long shadows being cast on the road, which vanish in the following shots. Their long shadows return when the convoy turns onto another road, but disappear again as they arrive at the main gate. (00:00:45)
Continuity mistake: During the siafu ant attack, while fisticuffs ensue between Indiana and Dovchenko, note the direction of Dovchenko's belt, with its buckle prongs to his left. Then, when Dovchenko falls backwards and the ants swarm all over him, his belt is worn around his waist the opposite way, with its prongs to his right (neither are flipped shots). (01:26:55)
Visible crew/equipment: During the jungle chase, Marion jumps out of the truck, leaving Dovchenko driverless. A few shots later, Spalko fires on the two amphibious vehicles, and when Mac removes his cap to check his head, the actor's modern wired device (earbud or mic perhaps) drops out from under his scarf/shirt and hangs over his shirt. Later, in the chamber with the crystal skeletons, Mac admits to lying about the CIA, then as Spalko climbs the chamber stairs, it looks like her lav mic is a bit visible between two upper buttons. Continuity mistake: When Indiana is with Mutt while the boy rides the motorcycle, Indiana's shoes change; note the soles change from heels to flat rubber-type soles, then back to heeled soles.
Continuity mistake: At Hangar 51, after the Soviets open the crate with the mummified alien, Indiana grabs his whip and secures guns for Mac and himself. In the close-up and wide shot, Indiana is gripping the whip within his palm, but in the next shot, the whip is held with only his index finger; then it's back to being gripped in his palm. Also, while Indiana is aiming the gun at Spalko, the military jeep behind him changes its distance, depending on the shot. (00:13:15)

Other mistake: In the FTI records room, when Arnold and Gerald open safe deposit box 999, the box is quite empty with a mirrored surface, and we can see the reflection of Scheck appearing behind them. The problem is Scheck's reflection is not backwards. (00:50:50)

Continuity mistake: Inside the FTI records room, Scheck tells Arnold and Gerald, "My guards will now lock you away where you won't cause any more trouble." As the FTI guards approach the boys, Gerald notices the two crisscrossed cutlasses hanging on the wall, with scabbards of different colors, but when Gerald reaches for those two weapons, the two cutlasses are facing the opposite way. Additionally, the lighter scabbard tip changes color, and the hooks they hang on also change, from transparent to silver. (00:51:50)

Continuity mistake: Scheck calls Nick and informs him the kids have a copy of the document on videotape, and they're headed uptown. During the following scenes, both Nick and Big Bob have medical adhesive tape on their cheeks (due to their earlier fight). While they're outside in the neighborhood, in some shots Nick's cheek has two crisscrossed strips of tape, but in other shots only a single strip of tape; the changes go back and forth. (01:03:20)

Continuity mistake: When Murray is told it's Mona's neighborhood that's to be demolished, he begins to speed. Murray then tells the kids to pray for the hunk of junk bus to hold together long enough to get them uptown in one piece. In the next shot, the rear right tire's hubcap flies off, but in the following shots, that hubcap is back on the rear right tire. (01:00:35)

Continuity mistake: After Scheck catches the duo in the FTI records room with safe deposit boxes, Scheck removes a painting from the wall while he rages on about the tomato incident; then he destroys the painting. However, when Arnold fights off the guard with the mustache, that painting is intact and back up on the wall in this shot. (00:51:30 - 00:51:55)

Continuity mistake: At the Grey Havens, just as Gandalf stands before his four brave Hobbits and says, "I will not say, 'Do not weep,' for not all tears are an evil," note the design on Pippin's scarf. When Gandalf says, "It is time, Frodo," the pattern on Pippin's scarf is entirely different, though it's the same style scarf. It then reverts back to the previous pattern when Frodo boards the White Ship.
