The Pyramid at the End of the World - S10-E8
Continuity mistake: When the Doctor, Bill, Nardole, et al. first see the pyramid their long shadows are cast directly in front of them, but when the Doctor decides to "bring it" and crosses the barrier, their long shadows are cast directly to their right. Then it keeps changing in the following shots when the Doctor approaches the pyramid. (00:10:45)

Continuity mistake: When Erica's eyeglasses get smashed, they're seen in a closeup inside her bag and the lens frame is cracked, but when she takes it out of her bag both lens frames are intact around the broken lenses. Also, when Erica takes the eyeglasses out of her bag she holds the eyeglasses' right temple in the closeup, but in the next shot it's the left temple being held up. (00:06:15)

Visible crew/equipment: Right before the TARDIS materializes in the lab at Agrofuel, there's an overhead shot of Erica at the desk, and the two green tape X marks on the floor behind her are visible, precisely where Douglas had collapsed and his white protective gear should be. Note there's an earlier overhead shot of the same area before the incident, when Erica brings Douglas the coffee, and those tape marks are not there. (00:32:50)

Continuity mistake: Clark and Bimbeau's squad car has a rear-view mirror that vanishes and reappears repeatedly throughout the movie.

Continuity mistake: When the boys are up in the tree peeping on Sally and her two girlfriends changing, their red car's license plate reads BAD480, but when Brenda and Shirley pull up to the drive-in their white car's license plate also reads BAD480.

Visible crew/equipment: Before Debra is taken away in the ambulance Dexter is physically stopped from getting closer to the scene, and when Batista tells the cop that Dexter is one of them, in the next shot facing Dexter the reflection of the moving overhead boom mic can be seen on the car window. Continuity mistake: After Gibbs burns the map in front of Barbossa it cuts to Jack waking up, and when he falls onto the floor this closeup is a flipped shot. Note the ring on Jack's right thumb which should be on his left, and all his hair trinkets are backwards.
Continuity mistake: After Carina tells Jack and the others that she's an astronomer which has nothing to do with donkeys, when Jack threatens to kill Henry in the shot facing him he's holding his left hand up pointing at Henry, but in the next shot facing Henry it's Jack's right hand up pointing at Henry.
Continuity mistake: Jack's knuckle tattoos on his right hand read "Jack" and the knuckle tattoos on his left hand read "Jakc" with the "k" at his ring finger and the "c" at his pinky. The ink intensity and letter style of his tattoos keep changing, and during the scene in the bank the "c" and "k" tattoos on his left hand have switched places, with the "c" at his ring finger and the "k" at his pinky.
Revealing mistake: When Jack stumbles out of the vault he questions the crowd as to why he's there, and as he holds up his left hand we see the tattoos on his knuckles, but when Jack says that he needs just a moment to clear his head, those knuckle tattoos on his left hand vanish and a ring appears out of nowhere. Then the tattoos reappear and the ring vanishes.
Continuity mistake: On the guillotine platform, when they strap Jack to the bench which will support his body, we see the double prong buckle is attached to the short leather strap beside Jack's right arm, and they slide the long strap with holes through the buckle frame. However, in following shots when Jack is on the ground the double prong buckle is actually attached to the long strap, and it's the short strap with holes positioned within the buckle frame.
Continuity mistake: When the sharks attack Jack and Henry's rowboat, one bites a chunk of the stern and the hammerhead bites a chunk of the bow. In following shots the bow bite vanishes as the shark leaps over Jack and Henry when it lands in the water, and the damage at the stern not only changes, but it also vanishes when the shark imitates Bruce from Jaws and lands on the boat.
Continuity mistake: While the Pearl's crew are battling Salazar's men, after Mullroy asks Carina if she's found what she's looking for and she shouts that she's trying, when it cuts to Barbossa fighting it's a flipped shot. Also, note that even though it's a flipped shot Barbossa still has a right wooden leg, due to CGI.
Continuity mistake: After Jack uses his spyglass to watch Will and Elizabeth reunite, when it cuts to Gibbs shouting, "Make way, Captain Jack Sparrow on deck," the shot of Gibbs and also the next six shots are all flipped shots, until Jack reaches the helm. In the first two flipped shots of Jack the beads and bone that are at the top of his head have been digitally altered to the right side of his head so the shots appear normal, but everything else is backwards.
Continuity mistake: After Barbossa releases the Black Pearl to her former and rightful glory, when it cuts to Jack and the others running on the beach the closeup and next long shot are flipped shots. Note that even though they're flipped shots Barbossa still has a right wooden leg, thanks to the CGI. (01:23:30)

Continuity mistake: When Clara takes Rigsy's chronolock, in the closeup of the tattoo Clara's lifting her hair with her right hand and showing the tattoo with her left hand, but in the next front-facing shot her hands have switched, she's holding up her hair with her left hand and showing the tattoo with her right hand. Continuity mistake: When Brackett leads Mr. Teal to his wife they walk into treatment 2, which has a windowless door, but in the shot from inside the room they're actually in treatment 1, which has a window on its door.

Visible crew/equipment: After Picard, Troi, and Worf have beamed down to the planet to meet Riva, when Harmony asks if they need food or refreshments, just as Picards says, "Thank you, no," a crewmember's elbow can be seen at the left side of the screen (this one is not visible on the Netflix version). Then when Troi says, "Your method of communication is elegant and quite beautiful," a crewmember (likely the same one as earlier) can be seen at the left side of the screen. (00:07:20 - 00:10:40)

Continuity mistake: After winning the semi-finals against 16, when Chet walks into the kitchen in the closeups John's holding the newspaper filled with photos on the page facing out, but in the wideshots an entirely different page faces out. (00:45:00)
