The Promise - S3-E11
Visible crew/equipment: Once the ledge rescue is over, after Johnny tells Brackett that he could put in a good word for him to join the paramedic program, when Roy opens the squad door to get in, the wing mirror offers up nice reflections of a few crew members.
Revealing mistake: After the two drivers from the traffic accident walk off holding hands, and Johnny tells Roy what happened with Barbara, while Roy explains "body-language" to Johnny, we can see the white tape with "Kevin" inside Roy's helmet.
Visible crew/equipment: During the structure fire, Johnny and Roy rescue the security guard, and when Roy breaks the window with the chair, we can see a couple of crew members, equipment, and lighting on a tripod, outside down below.
Visible crew/equipment: When Station 51 arrives at the salvage yard's fire, Johnny gets out of the squad, and when he puts on his turnout coat as the camera zooms out, a dark haired crew member wearing a short sleeve white shirt is visible at the lower right corner of the screen.

Visible crew/equipment: At the site of the major chemical leak, after Johnny falls from the pipes and Roy lifts him up, a couple of shots later there is a medium shot of one of 105's guys on Deluge 105 pumping out water, and we can see a cameraman (filming Deluge 105's closeups that we see during this scene) holding a handheld on the right of the screen, behind the pillar.
Visible crew/equipment: When Chet, Roy, and Johnny are with the mother and child at the scene of the vehicle accident, the reflections of location set array lights, four side-by-side reflector screens with some crew members holding them and the camera/cameraman are all visible in the window and surface of the funeral home hearse, as it arrives and then pulls to the side of the road.

Visible crew/equipment: After Roy and Johnny rescue the worker in the midst of the chemical leak, when Captain Stanley asks the Chief over the HT to send the ambulance, two crew members are seen behind him. When the victim is pulled across the lifeline, as he reaches the engine a crew member, with black hair wearing a white shirt, is seen standing behind the engine. Then, when Johnny hauls himself across the lifeline, just as he reaches the engine we see a crew member, with black hair wearing a white shirt (likely same one as earlier), who arches up then ducks back down behind the engine.
Propinquity - S3-E20
Visible crew/equipment: At the abandoned refinery, just after Roy attempts to pull out the man pinned under a tank, but is unsuccessful the first time, when it cuts to Captain Stanley and Chet, who are on the third level hosing the fire through the doorway, at the left of the screen a crew member is visible, watching them.
Visible crew/equipment: When Squad 51 is heading to 1511 West Hill, responding to the attempted suicide, in the closeup of Roy and Johnny en route, we see the reflection of Squad 51 as they're driving past houses/stores on Johnny's side. Just about as Roy says, "It should be it over there," in the reflection we see two crew members riding on the squad - one on top of the squad, and the other one standing on the back.
Other mistake: When John and Roy get back to the station, they head to the kitchen and John takes the hot pot of stew out of the oven with potholders, and when he quickly tries to lift the lid without the potholder, his pain makes it clear that it's burning hot, so he uses the mitt to lift the lid. There's a long metal serving spoon that was left in the stew, and John just lifts it up and starts serving the stew with no problem, even though that spoon should have been too hot to even touch.
Visible crew/equipment: When Roy and John are in The Boat with the victim of the explosion, the ambulance arrives and just as it passes by we are able to see a nice reflection of crew members, location set's 9-bank array lighting, reflector screens, a ladder and other equipment.
Propinquity - S3-E20
Visible crew/equipment: When Roy moves the squad forward to tow the wrecked car away from the flames, as he opens the driver side door to get out, we can see the reflection of the silver reflector screen in the squad's long wing mirror.
Inventions - S3-E22
Visible crew/equipment: On site at the major chemical leak, when Johnny burns his hand and falls backward, on the ground behind him there's a crew member that can be seen crouching behind stacked cement bags, looking in Johnny's direction.

Visible crew/equipment: When Roy and Dixie are talking at the nurses' station about the "radioactive" truck driver, Johnny walks up to them, and when he removes the rope and stanchion, the actor's tape mark is visible at Johnny's feet.
Visible crew/equipment: When the two ambulance attendants remove and roll the stretcher away from the ambulance, towards the stockbroker with chest pains, just as the words "Directed by Dennis Donnelly" appear onscreen, the moving shadow of the cameraman holding a handheld camera is visible on the ground.
Visible crew/equipment: After Captain Stanley's clam chowder lunch, the tones drop for the unknown type rescue, and when Cap hands Roy the piece of paper with the location, the reflection of a female crew member is visible on Squad 51's vent window, beside Roy.
The Promise - S3-E11
Plot hole: When Bo Jenson visits Richard Allen, he takes the elevator at the far south end of the ER wing, near the ER nurses' station, straight up to the 6th floor and then turns left to get to Richard's room in the short hallway, by the windows. When Richard jumps out the window and lands on the ledge, we see he's on the south side of Rampart's main building. Considering the actual floor plan of the hospital, this is completely impossible. The ER is located in the south lower wing and only has two floors, so Jensen could not have gone up to the 6th floor from the elevator he took.
Visible crew/equipment: At the structure fire, right after Captain Stanley and Roy throw the rope down to Johnny and the boy, and Cap tells Marco to use fog, in the next overhead shot as the guys use the axe on the partition fire, at the right side of the screen we can see a crew member wearing a short sleeve grey T-shirt, who's standing at the stairway landing right outside the door, as he's using the FX smoke/fog machine which is blowing smoke through the doorway.
Other mistake: When Tommy's on the ledge, and Roy and John are on the aerial ladder, just as Tommy jumps off the ledge and Roy catches him, we can see the young actor happy and grinning as he's being caught by Kevin Tighe, but within the very serious storyline of a 7 year old boy jumping from a ledge, looking "empty, tired and giving up", this doesn't fit. Obviously the young actor thought it was great fun to do a stunt like that.
Other mistake: When Station 51 is dispatched to the woman who fell through the ground into the abandoned well in her backyard, en route there's a moving bird's-eye view as the filming chopper follows 51's vehicles making a right turn. On the ground below, there are two people standing beside a blue station wagon, and one points up to the filming chopper flying overhead.
Answer: It is standard practice to wet down driveways so that they stand out in a long shot.
