Visible crew/equipment: At The Game, when Roy and John respond to the possible heart attack which turns out to be a hyperventilating man, his wife stands behind Roy, and the bulging outline of the lavalier mic transmitter is visible on the back of her yellow blouse.

Continuity mistake: When Roy's shaving while Johnny's in the shower, Roy fills a bowl with cold water and tosses it into Johnny's shower stall, then goes back to shaving, and the shaving foam on Roy's face changes between these shots.

Visible crew/equipment: At the start, after the boy ties a bowline knot in the rope attached to the van, when the mom gets into the van with her daughter, the reflections of two crew members, the cameraman and the top of the camera are all visible in the vent window, in two shots.

Revealing mistake: At the gas station, when the pump sparks and catches fire, we can see that it's not gasoline on the ground with the water from the fire hose, but instead it's floating thick gobs of stunt fire gel with burning flames.

Breakdown - S6-E15

Audio problem: When the guys reach the top of the tower ride at Oceanland Park, we hear Captain Stanley say, "Kelly, you go up on top and throw the tag line over the side, we'll take the stokes up that way," but Cap's mouth is saying something entirely different.

Breakdown - S6-E15

Visible crew/equipment: When Charlie's at the station, complaining that every time one of the squads breaks down it's only crying out for mercy against the drivers, and that firemen are the worst thing to happen to the squads, we can see the reflection of the boom mic on the windshield of Squad 51.

Other mistake: Before the fire in the patient's room, which is caused by liquid shorting out the clock radio, we can see the tape covering the brand names of the baby powder and the clock radio.

Bottom Line - S6-E17

Revealing mistake: While the guys are eating chilli at the table, when Station 51 is dispatched to 'man down', notice that when they're at the scene, both paramedics are wearing their jackets and Johnny is wearing his helmet. We never see Johnny's face in any of the wide shots, because the entire time it's the double with Roy. We only see Johnny alone in a few closeups, pickup shots which were filmed later, and we can see the style of the houses in the closeups do not resemble the houses in the other shots. The real Johnny is back, once they reach the hospital.

Firehouse Quintet - S6-E18

Continuity mistake: At start of episode, when Captain Stanley walks in announcing the upcoming game against 16, the paper he's holding changes hands between shots. Then after Cap sits down, the paper again changes hands between shots. Also notice that after Chet finds Henry's bone behind him in the couch, he gives it to Henry then puts it back behind him again, then gives it to Henry once again.

Firehouse Quintet - S6-E18

Revealing mistake: When Station 51 is dispatched to the gas explosion at the cafeteria, during the actual rescue the four firemen we see in closeup in the cafeteria are supposed to be Roy, Johnny, Chet, and Marco, but it's really odd that all are the doubles, very obvious in the closeups. There's little dialogue during this scene, which was dubbed later in ADR.

Continuity mistake: When Captain Stanley is writing on the board about the practice game at 9:00 am on Thursday, the board was clean before he started writing, then the board is dirty, and by the time he walks away the board is clean around what Cap just wrote, and his handwriting and its position on the board have changed as well.
