Character mistake: After the cat has her kittens, when Squad 51 is dispatched to the 'man trapped in boating accident', we hear Captain Stanley respond, "Station 51, KMG365," even though his response should have been, "Squad 51, KMG365," since it's only John and Roy that were dispatched, not the entire station.

Deliberate mistake: Rampart's ER is located in the south lower wing of the hospital complex and it is only a two-storey building (with a basement), but the elevator, which is located past the nurses' station at the wing's far south end, has two floor indicator panels listing eight floors - one panel over the elevator doors and another we see every time the elevator doors open, which is not possible.

Other mistake: After Battalion Chief tells Squad 51 to go to the elevator shaft, when Roy, Johnny and Jason enter the building, the DVD subtitles read that Captain Stanley says "Here, the elevator, it's jammed," when in fact it's the Battalion Chief who says that - Captain Stanley is not even with them. Additionally, Johnny asks, "Is somebody in here, Cap?" even though he's addressing the Battalion Chief, not Captain Stanley. Then it's Roy who asks for the oxygen, but subtitles read that Johnny asks for it.

Other mistake: When the dispatcher drops tones for the structure fire, he calls on Station 51, Engine 27, Engine 32, and Truck 105. Once there, Captain Stanley tells the dispatcher to send a second alarm, then hear him call on Station 19, Truck 98, Truck 34, and Battalion 43. However, the only ones at the fire with 51 are Engine 9, Engine 17, Truck 17, and two men with helmets from 20, even though dispatch did not call on any of them, and of all the companies that were called to the fire, none were there, except for 51.

Continuity mistake: When 51's guys are at the back of the station with The Old Engine, Johnny tries cranking it and his watch reads 5:20. Then after Squad 51 is dispatched to the heart attack, when Johnny's on the line with Rampart after defibrillating the patient his watch reads 11:15, then after administering atropine, his watch reads 2:30.

Continuity mistake: While Squad 51 is out on the run (quite literally) with the female OD, Johnny gets a cut on his elbow which he keeps fidgeting with even after it's bandaged. When Roy and Johnny get back to the station and see The Old Engine, Johnny's cut has vanished.

Character mistake: When Hank is brought to the hospital with gunshot wounds, the ambulance and its two EMTs are met outside by two doctors. One doctor says "Let's get him two grams of oxygen. What's his hemoglobin?" This episode's technical adviser was perhaps away for this scene. How much exactly is two grams of oxygen? Orders for administering oxygen would be given in liters, such as two liters per minute (LPM). The line is presumably meant to be 'two liters of oxygen'. (00:02:25)

Continuity mistake: At the station, after Johnny has searched through a bunch of papers for the receipt for $8.42, when he gathers them up a yellow paper falls to the floor at his feet, just before Roy suggests putting them in alphabetical order. Then after Johnny stuffs the papers in an envelope and tosses the envelope in the garbage, the paper that fell on the floor has vanished.

Other mistake: After the apartment house fire, the restored old engine is trashed by a hail of bricks. Later, when Roy and Johnny are talking about who won the first prize, The Old Engine in the back looks nothing like it did when it was wrecked on the street. Also, the shot of The Old Engine that's shown here, is actually the same footage shown in season 3, "The Old Engine", with the lighting dimmed to signify nighttime, when at the end of the episode Roy and Johnny decide to keep it. Note the exact same broken pieces of windshield, tires, hubcaps, dirt, shadows, everything.

Continuity mistake: When Johnny and Roy walk into Councilman Wilby's home, Wilby's wearing a buttoned up shirt and tie, and they place the O2 tank near his feet, but in Wilby's first three closeups the O2 tank is already behind the couch by his head and his shirt is unbuttoned with no tie, but in the wide shots Roy and Johnny haven't even made those changes yet.

Continuity mistake: When Brackett is first on the line with the vet, there's a closeup of Early checking the goat's heartbeat and we see the baby goat has been intubated. Brackett informs the vet of the goat's heart condition, so the vet tells Brackett to mask him with halothane and intubate. Then a minute goes by before the vet says that they should have him intubated by now, and when Brackett gets back on the line he says that the animal's intubated, even though we saw the animal already intubated in the closeup right at the start.

Other mistake: When Brackett tells Johnny and Roy to contact the animal shelter via the disaster net, Roy asks the Animal Control guys for their disaster call, and it's given as 3-9-9-7-7-1-1, but in the closeup Roy presses channel 8 then punches in 4-2-1-6-5-3-7 on the scanner. Additionally, this model Regency Monitoradio / Executive Scanner Receiver has 8 channels, so it would have been impossible to press 9, as was stated by Dave.

Character mistake: In the morning, when Roy and Johnny are by the lockers, talking about what Johnny did on his days off, Roy tells Johnny, "I came on this morning, guys on A shift told me that they had a meeting with Brackett at Rampart yesterday." It could not have been "A shift" because Roy and Johnny work the squad for A shift, with Captain Stanley and the rest of the guys. And we know that Roy and Johnny didn't switch shifts with B or C, since the rest of A shift are there with them, working the engine that day.

Character mistake: When Brackett and Early are dealing with three squads at the base station at the same time, Squad 51 has two victims - father and son, Squad 18 has a pulmonary edema, and Squad 36 has only one bicycle rider with severe head injuries, because the second rider died, so the total is four out of five being dealt with. Back at the station, after Johnny asked Dixie about the condition of all the victims, he tells Roy that the father and son are both responding, so is Squad 18's pulmonary edema, and for Squad 36's bike accident, that "both" are coming out of the coma, which totals five victims. Then Roy responds, "Four saves out of five victims. That's pretty good." Johnny should not have said, "both" since it was just one rider.

Character mistake: Days after Drew's death (Roy and Johnny had two days off after Drew's death), during the meeting that Brackett has with the four paramedic units, one of 59's guys asks, "What do we do when we have this overload situation on the same frequency? Two squads, like 51 and us today?" The situation with the frequency did not happen "today" as he said, it happened at least three days prior.

Continuity mistake: After the Bengal tiger jumps on Les, the feline walks up the aisle that is directly in front of the self-serve ice cream freezer, and knocks over the pyramid of Hi-C cans that block the aisle, but a few shots later the tiger walks up that same aisle (note the ice cream freezer), and the mess of Hi-C cans have vanished.

Other mistake: After the officer hands Les the tranquilizer gun, Les asks which way the tiger went and the officer responds, "Down that aisle," and we see both men, who are standing right beside the self-serve ice cream freezer, moving slowly towards the aisle in front of that ice cream freezer. The problem is that in the next shot, we see the large tiger coming from that exact area in front of the ice cream freezer, as he walks toward the camera on the other end of the aisle. This means that the Bengal tiger, Les, and the officer would have all been standing in the very same spot, at the same time.

Continuity mistake: After Johnny sees the Bengal tiger in the market, he backs away, and when Johnny is about to walk through the curtain, in the shot from the back room we see the tall shelving display of chip bags on the other side. However, in the next shot from the store that display of bags is gone, there's only a display of loaves of bread in front of the fruits and vegetables.
