Revealing mistake: When Tony goes to the bathroom and scans his blood toxicity, the Tony who faces the camera is supposed to be his reflection in the mirror, but the problem is the reflection is not reversed because it's not a mirror image, note Tony's clothing. Also, note how he scanned his right hand's finger, but then in the "mirror" he's sucking on his left hand's pricked finger. (00:28:30)

Deliberate mistake: On Tatooine, after C-3PO tells R2, "No more adventures," he walks off in the opposite direction. When C-3PO reaches the skeletal remains in the sand, and complains that the malfunctioning little twerp tricked him, the closeups of 3PO are flipped shots (also note the position/direction of the skeletal head). Visible crew/equipment: After Chris exits the TARDIS with the medical kit, Romana places the collar around the Professor's neck. When Romana tells Chris that Chronotis, "Should be able to think with it," in the next closeup of Chris the moving boom mic is visible at the top of the screen, before he kneels down.

Visible crew/equipment: (Part 3) After Omega has revealed himself to The Third Doctor, Jo, and Dr. Tyler, when he begins to speak privately to The Doctor the camera casts a shadow on Omega's body. Then after Omega turns around the boom mic dips into view at the top of the screen. And when Omega starts walking forward, the shadow of equipment (presumably the boom and mic) is moving on the far wall, near the lower left of the screen. (00:03:10)

Revealing mistake: While Julius is talking to the man in front of the lingerie store on Hollywood Blvd, when the man's partner rides up on a motorcycle and tries to snatch the suitcase, we can see the stunt wire carabiner attached to the back of his jacket as he flies off. (00:12:30)

Visible crew/equipment: After Sykes shoots the Transit Cop on the "L" Richard pulls the emergency brake, and in the following shot facing Richard as the train screeches in flashes of darkness, the reflection of a crew member quickly becomes visible on the glass window beside Richard. (01:47:45)

In this and following long shots of the stunt, we can also see the window roll cage bar directly behind the front windshield (the obvious stunt driver grips the roll bar with his left hand and steers with his right hand), as well as the top of the seat supports behind the front headrests. Visible crew/equipment: After Tom tells President Palmer that Sandra has been arrested, when it cuts to the interior shot inside the car with Agent Samuels in the front passenger seat, the camera pans toward the rear passenger window showing all the protesters outside. In this shot there's a reflection of the camera operator's leg, wearing shorts, white socks and sneakers, and who is sitting where Sandra should be.
