Continuity mistake: When Ricky begins walking out of the assistant warden's office after first meeting him, he holds up a photo of his girlfriend and tells him to keep his nose out of his affairs. Some blood can be seen on the back of the photo. In the very next scene, Ricky is shown sitting in his cell looking at the photo, with no blood on the back of it. (00:27:24)

Continuity mistake: Just after Ricky has broken the assistant warden's desk and dodged a bullet fired at him, there is a shot of Ricky wiping off a small amount of blood on his face, tasting it, then clenches his fists and advances on the assistant warden. Here we can see a small droplet of blood on his stomach. But in the next shot, when he stops charging toward in the assistant warden, the blood on his stomach has vanished. (00:27:00)

Continuity mistake: When Ricky breaks the assistant warden's desk after getting stabbed through the hand, in the shot of the desk collapsing, strips of tape can be seen on both the left and right hand side of the table. He breaks a small corner on the headstone with his hand to demonstrate, however it can be noticed that there are visible cut marks on the corner that he breaks off before doing so, revealing that the corner had already been cut off and reapplied to make it easier for Uncle Ghost to chop it off. It can be noticed that the man is just holding the train in his hand whilst falling and drops it when he lands on the ground to make it appear as if it fell out of his clothing. He was not meant to be holding the train as it was not shown being held in his hand at any previous point. (00:07:25)

Continuity mistake: When the two prison guards discover that there is metal inside Ricky's chest, they begin pushing him over to the X-ray machine. The guard holding a baton is shown pushing the bottom of the baton against Ricky's upper back, but in the next shot, he's suddenly pressing the entirety of the baton against Ricky's back. (00:05:39)
