Graduation Match: Part 2 - S1-E52

Plot hole: When Zane has Cyber End Dragon attack Rampart Blaster, Jaden activates De-Fusion, allowing Rampart Blaster to be sent back to his deck (and allowing Burstinatrix and Clayman to come back to the field). Later, when Jaden summons Shining Flare Wingman, he states that it will gain 300 points for each Elemental Hero in his graveyard. We are then shown eight Elemental Heroes in his graveyard, including Rampart Blaster, but this card was never sent to the graveyard, it was sent back to his deck. (00:09:13 - 00:14:43)

Merry Christmas Charlie Manson! - S2-E16

Continuity mistake: When Cartman tells Elvin that he finished his fudgesicle, the two are standing right next to each other. The shot then cuts to focus on Elvin crying, where Cartman cannot be seen next to him. A couple seconds later, when Cartman hits Elvin over the head, the two are right next to each other, but in the shot of Elvin, Cartman cannot be seen next to him.

Tag Team Trial: Part 1 - S1-E10

Stupidity: Para uses a spell to summon Suijin (2500 ATK), he sacrifices Kaiser Seahorse to summon Sanga of the Thunder (2500 ATK). Then sacrifices those two monsters, along with Kazejin (2400 ATK) to summon Gate Guardian (3750 ATK). He then has Gate Guardian attack Steam Gyroid causing Syrus to lose 1550 points. But if he had attacked Steam Gyroid with one of the monsters he already summoned, then attacked directly with the other, Syrus and Jaden would have lost significantly more life points.

Graduation Match: Part 1 - S1-E51

Other mistake: Zane activates the spell card Future Fusion. With this card, he is able to send three Cyber Dragons from his deck to the graveyard and summon the Fusion monster Cyber End Dragon to the field. However, this is not how Future Fusion works. When this card is activated, the player must wait two turns until the Fusion monster is summoned, it can't be summoned on the turn that it is used. (00:17:15)

Other mistake: When Jaden places Elemental Hero Burstinatrix on his duel disk, the Elemental Hero Bubbleman card can be seen next to it. Bubbleman is shown to have 700 attack points listed on the card, even though this card is shown to have 800 attack points every time it is used in the series. (00:16:55)

Formula for Success - S1-E12

Plot hole: Bastion uses the special ability of Hydrogeddon, where he is able to summon another Hydrogeddon from his deck. He summons this monster by drawing the first card from the top of his deck, but there would have been no way for him to know if the card on top of his deck was a Hydrogeddon card. (00:12:35)

Graduation Match: Part 1 - S1-E51

Stupidity: Zane activates the spell card Polymerization and explains that "by sending some select monsters from my hand to the grave, I can summon a Fusion monster", to which Jaden replies "To the grave?" as if he is completely surprised by the fact that the monsters will go to the grave. Jaden is completely familiar with Polymerization and has used it in nearly every duel, so he should be aware that the monsters will be sent to the graveyard when Polymerization is used. (00:07:42)

Doomsday Duel: Part 2 - S1-E30

Other mistake: Jaden activates the spell card Silent Doom and summons Elemental Hero Sparkman to the field in defence mode. He then activates the field spell Fusion Gate, but when he goes to insert the card into his duel disk, Sparkman is shown at a portrait angle on the duel disk, indicating that it is in attack mode instead of defence mode. School Duel: Part 2 - S1-E26

Other mistake: When Jaden has Elemental Hero Wildheart attack Chazz directly, Chazz's life points go down to 2100. Chazz's life point count remains this way for a large chunk of the duel, but when Elemental Hero Flame Wingman destroys Armed Dragon LV7, Chazz's life point count is 2800. Technically speaking, Chazz does lose the correct number of life points since his go down to 1600 and there was a 500-point difference between the two monsters, but his life points shouldn't have been listed as 2800.
