Continuity mistake: Just after the warden has chopped off the inmate's hand by forcing it into the meat grinder, the inmate falls over and some of the blood coming out of his arm splatters on the wall beside him. Two other inmates walk up to him to help him up, and now there is suddenly much more blood on the wall. The blood also changes appearance, being splattered on the wall in one shot, then being smeared on the wall in the next shot. (01:22:16)

Revealing mistake: When White God fires a needle through the wall of the kitchen, Ricky grabs onto it and is dragged towards the kitchen wall. Just before he crashes into the kitchen wall, a large outline can be seen in the wall, revealing where the effects team made a hole in the wall and plastered over it before filming. (01:23:38)

Other mistake: Just after the warden tells the guard to limit the amount of food the inmates get in their meals, one of the inmates shown polishing the meat grinder says "You eat like a lousy king. Even your dog gets a big steak." When he says "your dog", the subtitles read "yourdog" instead of "your dog." It has been incorrectly spelt as one word, with no space shown in between the two words. (This depends on which format and which studio distribution you are watching, but it can definitely be seen on the Region B Blu-Ray distributed by MediumRare). (01:21:53)

Other mistake: Just after the assistant warden has been carried away by the other inmate, it cuts to the warden in the kitchen with his son and a guard. When the guard tells him the menu for tomorrow is ready, the subtitles "Sir, the prisoner's menu for tomorrow is ready." The apostrophe in "prisoner's" is shown in the wrong place. Saying "prisoner's" implies that it's just for one prisoner, and since the menu is made for all of the prisoners, the subtitles should have read "the prisoners' menu" and not "the prisoner's menu." The guard even says "They'll have some beans, vegetables and meat for lunch" afterwards, so multiple people were being referred to. Other mistake: Just after the assistant warden's right arm has been chopped off by an inmate, part of his shirt sleeve goes missing. The part of his arm still attached to his body has a sleeve starting at his shoulder, and stopping an inch above where his arm stump ends, leaving an inch of skin exposed. And when the chopped off arm is shown, the arm is shown to have a sleeve starting at his elbow, going down to his wrist, but there are a few inches of his upper arm above his elbow left exposed. This means that a couple inches of his sleeve from where part of his upper arm was located, have gone missing without explanation. (01:20:25)

Continuity mistake: When the armoured guards appear, a guard holding a mace attempts to swing it at Ricky whilst he jumps from a staircase. In the shot of Ricky jumping down from the staircase, he is holding his shield so that it is positioned away from him, but in the next shot, the guard suddenly has his protective shield directly in front of him. Revealing mistake: Just after Ricky has killed the bald inmate, he then goes to punch the assistant warden in his face, resulting in his one real eye falling out. In the close-up shot of the assistant warden getting punched, a fake prop dummy can be seen instead of the real assistant warden, most likely used so that it would be easier to create the practical effect of his eye falling out. Continuity mistake: When the assistant warden falls down the stairs whilst being confronted by the inmates, in one shot his body is rotated towards the left and is face to face with some inmates nearest to the staircase. When he notices Ricky in the next shot, the left door on the toilet stall is now closed and the right door on the toilet stall is now wide open. (01:19:09)

The inmate standing on the left has both of his hands on the bars and the inmate standing on the right has both hands on the row of bars positioned below where the inmate standing on the left has his hands. When the camera changes angles, the inmate standing on the left no longer has his hands on the bars and the inmate standing on the right now has his hands positioned on two separate rows of bars. Continuity mistake: When Ricky is being tortured by the warden, after the warden removes the tape covering his mouth, Ricky spits out some blood as many of the razors inside his mouth. The razors that were shown sticking out of his cheeks are not shown moving, but after it cuts to the warden with some of the razors stuck to his face, the shot cuts back to Ricky and the razors in his cheeks are no longer there. Even the ones that weren't shown being spat out. (01:16:40)

After the warden asks Ricky a question and Wong Chun stops hitting him for a moment, we can see that Ricky's left cheek looks fine (aside from the blood running down his head). In the next shot, the razors are suddenly sticking out of his cheeks that were not there in the previous shot. (01:16:27)
