Continuity mistake: When the movie starts, Edward and his assistant hold a cup of coffee, with the handle facing 6 0'clock. From a different angle, half a second later, the handle is facing the opposite direction, without anybody moving the cup.

Visible crew/equipment: Edward arrives at this daughter's house and two big white lighting screens get reflected on the car's door.

Audio problem: In the hospital, while discussing the list, Edward talks to Carter and, from the back, he says the word "grief" and the sound stops; but his jaw keeps on moving as if he kept on talking.

Continuity mistake: The amount of sacks by the oven that Laura moves away keeps increasing and decreasing randomly all the time.

Revealing mistake: When Laura falls in the bathtub, a stunt is doing the fall, because her hair colour and style changes.

Continuity mistake: The hacker slaps McClane's daughter moving her head to the right, hair messy around her face. The immediate angle from the hacker's point of view shows her head tilted to the opposite direction, with her hair perfectly brushed.

Deliberate mistake: When John crashes the car against the elevator room, the cables he and the woman hold on to are clean, when they should be greasy - thus allowing for them to hold safely.

Continuity mistake: During the traffic jam scene, when the truck crashes against the taxi, the view inside McClane's car shows the taxi colliding against the car, dragging it away violently. From the aerial angle, the taxi doesn't touch McClane's car at all. (00:22:35)

Continuity mistake: Mike enters the hotel and a woman in the back, sitting next to the woman with the pram, disappears between shots.

Continuity mistake: Mike fixes the boat painting, his hair and face are dry. He turns around and sits down and now he is all sweaty and with messy hair.

Continuity mistake: When Mike reaches the phone and hears the operator asking if he is checking out, the phone on the table swaps from facing down to up between shots.

Continuity mistake: When rain starts to pour in the frozen room, Mike's positions swaps from next to the television to 3 meters to the right in half a second.

Continuity mistake: After Mike enters through the window, he lays down and his left arm is extended up to his head. Depending on the angle, in about 2 seconds it is in different positions, without him moving it.

Continuity mistake: When frozen Mike opens the laptop the lamp behind keeps moving around between shots.

Continuity mistake: When frozen Mike crouches towards the laptop he takes the blanket off of him. In the next angle the blanket is again on his back.
