Visible crew/equipment: Boom mics, crew members, equipment and lighting screens are reflected constantly on the actors' sunglasses throughout the movie (during the fresh clam terrace scene, when Augie and Ginger tell Jasmine and Hal about the lottery, when Augie and Ginger have a picture taken next to a yellow boat... etc.).

Continuity mistake: Josh stumbles into McMillan, scattering papers everywhere. Susan picks up hers and presses them against her chest. In a close-up they're tidy, yet a shot later they're all messed up.

Continuity mistake: When Walter White is with the Neo-Nazis waiting for Jesse to appear, one of the thugs throws Walter's car keys and wallet on the pool table. Their position changes between shots, sometimes being parallel to each other, other times very far apart. (00:40:15 - 00:42:10)

Revealing mistake: When McClane is hanging on the steel cable which pulls their car to the river, right when he says "Run, run" there's a brief shot and it's very obvious that the guy is Bruce Willis's stunt double.
