Stupidity: After Paul is shot, he takes medical supplies from the hospital to his office. However, he doesn't think to take Lidocaine, or some other local anesthetic, knowing that the procedure will hurt.

Continuity mistake: When Fish gets shot in the leg and runs out of ammo, we see him on the monitor still trying to shoot in vain. When it cuts to him, the slide is locked back and we see him release the slide and try to shoot. But in the next shot of him, the slide is once again locked back. Since we know he's out of ammo and he didn't reload or fire the gun, the only way for that to happen is if he manually pulled the slide back, which he didn't have an opportunity to do (not that it would even make sense to do).

Stupidity: Eve, the assassin trying to kill the Senator, tells Cole to move or she'll shoot through him, rather than just shooting him. She already killed an agent by shooting him in the back and had plenty of time to just shoot Cole, so there's no reason for her to avoid killing him.

Continuity mistake: When the family is in the room, the bookmark in the Tom Clancy book is sticking out when Riana is holding it and even when Trish hands it to Roger, but when we see him with it, the bookmark is barely visible and not sticking out.

Trivia: When signing the International Superhero Accord, all the Ambassadors, save Henrietta Selick, are name of various crew members that worked on the film. For example, Alan Broiler was the supervising animator. That said she's clearly a reference to Henry Selick, a graduate of California Institute of Arts like director Brad Bird, who went on to direct The Nightmare Before Christmas and Coraline.

Continuity mistake: At the school, when Scott's suit is malfunctioning and he's big in the janitor's closet, his head is tilted towards his right shoulder. When Hope is working on the suit, we see Scott's head is now tilted towards his left shoulder, even though he's stuck and wouldn't have been able to move around like that. But in the next shot it's back to his right.

Common mistakes

Character mistake: When someone tries to call another person's cell phone in an emergency situation and it goes to voicemail, but they just keep calling over and over (with or without leaving a message). They never try sending a text message or similar, they just repeat the same pattern despite knowing the other person isn't answering.

Character mistake: In the opening scene, Sherlock makes a point to correct Barry's poor grammar. But later, when talking to Watson, Sherlock uses poor grammar by saying "who's sleeping with who." It should be "who's sleeping with whom." Not a mistake Sherlock would make.

Question: Matt has the taxi take him to the prison and tells the driver to wait for him. But the taxi is parked right next to the exit gate, so as close to the prison as you can get. During such an intense riot and lockdown, would a corrections officer tell the taxi to move? This is a question for people with prison/correctional work experience in regards to lockdown procedures.
