Continuity mistake: When McClane is driving the police down the ramp to the tunnel so he can elude the helicopter, the car's windshield doesn't have any bullet holes or cracks in it, even though it did just a few seconds earlier. (00:37:55)

Continuity mistake: When Jerry scratches a '5' with his watch onto the wall, in a wide shot he starts to scratch the 5, then in the closeup of him scratching it onto the wall, much more of the 5 has been completed between shots. (00:17:35)

Continuity mistake: During the traffic jam scene, when the truck crashes against the taxi, the view inside McClane's car shows the taxi colliding against the car, dragging it away violently. From the aerial angle, the taxi doesn't touch McClane's car at all. (00:22:35)

Continuity mistake: When they start running from the moving wall Jerry grab Sasha holding her left hand. When it cuts he is holding her right hand. (00:20:05)

Continuity mistake: When McClane and Farrell are fired upon during the escort to DHS, Agent Johnson is killed and slumps against the steering wheel, and in the next shot the deceased's foot slips off the brake so the car jolts forward and hits the FBI's SUV in front of them. Four shots later, in the interior shot from the back seat directly behind McClane ducking inside the car, we see Agent Johnson sitting upright very much alive, at the left side of the screen. Continuity mistake: Once Grandpa takes the underwear off Harry's head and sets it on the counter, it continually moves. Originally it is at the edge, draped over a little bit. Next it moves inward a little. After Mom shuts off the power, it is almost halfway on the counter. (00:11:40 - 00:13:00)

Revealing mistake: When Kate decides to ditch Maguire and escape herself, we get a shot from above showing her face and Maguire's feet. However we only see his trousers. His feet are gone. Even though some could say that the wall removed them, but we can also see that the wall hasn't reached the part of the feet that are missing. It appears the CGI team forgot to add his feet in. Continuity mistake: When Powell is driving his cruiser in reverse to get away from the bad guys shooting at him, several shots show a full roll-cage in the car which is not there in any other scene or close-up of the interior. (00:55:10)

Continuity mistake: In the traffic jam in the beginning of the film (right after all the lights have changed to green), the car next to McClane's is black/dark. But a few seconds later when you see it from a helicopter, it has changed to white. (00:22:30 - 00:23:30)

Continuity mistake: At the end of the scene where Bruce Willis's own car is involved with a crash between a dustcart and taxi you can see his driver's door opening during the aerial shot after he has left the location. (00:24:25)

Continuity mistake: After he climbs out of the air duct, John McClane's shirt goes from white to green. The dirt and grime from the air duct would cause discolouration only where the shirt comes in contact with the duct. However, his shirt is uniformly green including places that wouldn't make contact and it is impossible for all surfaces to make contact even in small areas. (00:23:05 - 00:56:30)

Continuity mistake: When Rachel and Phoebe are talking in the Baby Beauty Contest and Rachel is holding Emma, Emma's head instantly changes position between shots several times, often going through 180 degrees. Clearly they couldn't get her to do the scene in one take and therefore the editing suffers. (00:07:05)

Continuity mistake: In the one where Rachel is in the hospital after having Emma she is in the bed feeding Emma and Ross is sitting right beside of her. They are talking about their relationship and what should come. If you will watch each shot as they go to showing Rachel's face then Ross's you can see that Rachel's hair in one shot is in front of her shoulder and the next shot it is behind her shoulder. This happens several times. (00:20:55)

The One Where No One Proposes - S9-E1

Continuity mistake: In the scene where Rachel believes Joey has proposed to her at the hospital after having Emma. Ross is talking to Rachel about her saying yes to Joey's proposal. The nurse brings Emma in to breast feed. When the nurse leaves, she closes the door. However, when they shoot back to Joey a couple of times before he leaves the room, the door is open. (00:19:45)

Continuity mistake: When Tess is cutting the underwear off of Harry's head, she leaves the scissors on the counter away from the pile of the pieces of the underwear. Later you can see that it is in the pile of underwear. (00:11:25)

Continuity mistake: The 1st scene where Harry blows a horn in Anna's ear, she chased him out of the room and her blanket was thrown over on the RIGHT SIDE, but if you look close when she goes to close the door the blanket it almost all the way off the bed on the LEFT SIDE. (00:01:35)

Continuity mistake: When Jim is about to play the trombone he has it nowhere near his shoulder. Camera cuts and he has already got it up by his shoulder. (00:30:55)
