Continuity mistake: When Scott wakes up after falling off Anthony, we can see the floor around the bed and there are no ants there. He then looks down and sees the bed is surrounded by ants.

Visible crew/equipment: Right when the Nazis are struck with the light, one soldier shakes wildly and one can see the cable coming out of his trousers and stretching off-screen to the right. (01:47:30)

Continuity mistake: When Scott uses the suit for the first time, he is on a tube with a small red towel on it which disappears between shots. Not only this - the amount of food on the plate reduces and then increases during the scene.

Revealing mistake: When the Nazis are about to glow (and die), check the one on our left. His shirt is already unbuttoned right where the light will subsequently glow (which was a mere light bulb). A second later, he triggers a device clutched in his right hand (seemingly a switch) seconds before his chest lights up. (01:47:25)

Continuity mistake: When the psychic is about to crack an egg, the cigar changes positions between shots. The ash also changes colour.

Revealing mistake: When the cop by the vending machine is about to be killed by the T-1000, the back shot of him reveals he's been replaced by an obvious mannequin. His perfectly trimmed hair along the nape is now long and messy, and a long stitch appears along his sideburn and down to the neck.

Continuity mistake: Indy horseback rides past a truck with a flag, then between a dozen of musicians while chased by a motorbike. When he looks behind him, the band has vanished and the truck is next to him again. This last shot belonged in the first place.

Continuity mistake: Indy stops Basil from hammering the dial and makes the left side of his jacket open to reveal his shirt. Shot changes, and now the opening has swapped sides.

Factual error: The crown on the logo of the Air Maroc plane is the modern 2013 version, not the one from 1969 when the movie is set.

Continuity mistake: While on the boat, Helena does her 7 spades card trick. When she gives Indy a turn, you can see the bottom card is a black 2. When she fans them out, it changes to black king of clubs. Then, in the next shot, it changes to 8 of clubs. (01:16:00 - 01:17:00)
