Audio problem: When Gemma is talking to Clay in jail there's a shot "en profil" in which we only see the silhouette of her face. When Clay sits down you can see she's saying something, while the only sound we hear is Clay exhaling.
Audio problem: When Carter is searching Jean Jackson's house she asks him if he has a warrant - although the camera isn't on her you can see her reflection in the mirror and you can see that her mouth doesn't open when she is asking Carter this.
Audio problem: The director can be heard saying "Action" at the start of the scene. (00:16:35)

Audio problem: When Easy are attacking the 88 battery, in the scene where Buck scouts out what's ahead of him before throwing grenades, you can see a soldier holding an MP40 (machine pistol), who fires one shot with the sound effect of a Kar98 rifle. (00:31:50)
Any Old Port in a Storm - S3-E2
Audio problem: Pay phones always dropped your coin into an internal cash box when you finished your call. In the bar, Columbo makes two calls, and after the first, we hear the coin drop as it should. When he finishes the second call, we should hear that coin fall, too, but it doesn't. (00:34:05)
Destiny (2) - S1-E22
Audio problem: At the end of the episode, Michael suddenly appears next to Max without any footsteps on the gravel like Max's and Liz's. (00:41:35)
Audio problem: Volkswagen Beetles had a very distinctive "putt-putt" engine noise. In every scene with the bad guy following Ryan and McBride in a VW Beetle, the sound effects dubbed in post-production belong to a much larger, standard American car, and not to a VW at all. (00:26:00)

Audio problem: When Kindaichi understands the trick and runs away from the murder room, Miyuki as usual exclaims "Hajime-chan!" but with a curious flub in animation, her tongue moves but not her lips. Try to say "Hajime-chan" keeping your mouth open all the time, it's fun. (00:18:45)
Audio problem: In the "previously" part, Grace whimpers "Harvey..." with a second of delay between sound and lip movement - a gap that was practically non-existent in the actual previous episode. (00:01:30)
The Spores - S2-E7
Audio problem: When the flower delivery van flies off the asphalt road and into the dirt, a tires-screeching-on-asphalt sound is heard as it hits the dirt before rolling over.
Episode #2.9 - S2-E9
Audio problem: The FBI interview Wayne Williams on the bridge and then decide to release him. The sound of the engine starting is heard as Williams is still getting in his car.
Suggested correction: That was Jim's car starting first so it could back up. The second car starting was that of Mr Williams after he was in the car.
Audio problem: In the last scene of the episode Sheridan says to Mr. Sebastian "The morning after the last of a string of murders in the East End." His lips however, say West End instead of East End. The script and initial broadcast used the words East End, but after it was pointed out that this was factually incorrect, the line was dubbed over for all subsequent broadcasts and releases. Audio problem: Just as Zen is reporting that it can find no stars for navigational reference points, something crashes very noisily off screen. No, it isn't the arriving Dayna's clunky shoes. Much too loud, unless she's throwing them at something.

Audio problem: When Joey is saying "Frankie, tag me," his mouth movement is not synchronized with the dialogue.
Pay in Pain - S1-E7
Audio problem: When Mackey is interrogating Leon the camera is pointed at Mackey as he asks Leon "why are you and the other shop owners all telling the same lie?" The camera then switches over Mackey's right shoulder so we can see Leon's reaction to Mackey's question. Look closely at Mackey's mouth in this shot, you can see him mouthing the exact words that he just spoke.
