Continuity mistake: When Klinger enters Potters' tent, the bird alternates from about waist height to chest height between shots.
Continuity mistake: When the pit boss is taking the rolled up 5 containing the pair of dice out of his pocket, his fingers are hiding the end toward Mulcahy. When the camera angle changes, the fingers are moved so Mulcahy can see the hidden dice.
Revealing mistake: During the melee at Rosie's, after Klinger gets his money and jewelry back from the cheats a guy is thrown through windows, and though we hear the sound of smashing glass we see the shreds of plastic fluttering.
Factual error: When the camera looks at Lt. Clark, her rank insignia is horizontal. It should set from the apex (point) of the stitching diagonally (top end toward her face).
Continuity mistake: As Charles talks to the guys in the Swamp, his right hand alternates from up at his chest to down by his side several times too quickly to be normal.
Factual error: While eating, General Haggarty was told about how Conway should be a cook. Haggarty said "Change his (Conway's) MOS. MOS means Military Occupational Specialty, your job in service. Since M*A*S*H was set in the early 50's, Haggarty wouldn't have said that, because 'MOS' wasn't used until 1965.
Suggested correction: "MOS" most certainly was used during the Korean War. In 1965, the MOS system was simply changed, not created.
I perhaps misspoke. It was titled PMOS, and revamped in 1965 as just MOS. My apologies and thanks for the info.
According to "Introduction to Special Regulations SR 615-25-15, Enlisted Personnel Military Occupational Specialties" from the Department of the Army in November 1950, it's MOS. The "P" just means primary, but MOS was/is used way more than PMOS.
Visible crew/equipment: When General Haggerty walks into the mess tent and stands beside Colonel Potter, the chalk T-mark can be seen on the floor near his feet, and when Haggerty wants to meet the cook the chalk mark can be seen on the floor where Conway stands.
Continuity mistake: While Father Mulcahy, Margaret, BJ, and Hawkeye are in the mess tent, when Colonel Potter walks over both of his shirt pockets are unbuttoned, but when he sits down the right pocket's buttoned up.
Visible crew/equipment: After BJ and Hawkeye have tasted the Spam Parmesan, when Klinger calls the cook over, in the shot of Conway the edge of the tent set can be seen at the top right corner of the screen.
Continuity mistake: After BJ and Hawkeye taste Klinger's plateful of Spam Parmesan they all sit down at the table, and when BJ pulls the plate away from Hawkeye it cuts to another angle and the knife has jumped to the other side of the plate, and the fickle cup keeps sidling across the table to be beside the bowl depending on the angle.

Visible crew/equipment: When Williamson and Hawkeye walk into the Swamp and stand near BJ's cot, the edge of the tent set can be seen at the top, left side of the screen.

Factual error: Then camera Klinger is using is not period correct. Those Brownie cameras were not around at that time.

Visible crew/equipment: When Williamson, the Congressional Aide, is in Colonel Potter's office questioning Margaret about Wally, right after Hawkeye makes the comment about liberty and justice for all, in the next wideshot as Williamson walks to the other side of the room the T-marks and chalk marks can be seen on the floor.

Visible crew/equipment: When Radar arrives back at camp the Korean driver fixes Radar's stiff neck, and just as Radar gives him the money and we hear people cheer that Radar's back, the shadow of the boom mic can be seen on the ground at the bottom left side of the screen.

Visible crew/equipment: When Radar gets back to the 4077 and goes to the office with Klinger, just as he walks into the doorway the shadow of the moving boom mic can be seen on the wall at the right side of the screen.

Visible crew/equipment: After Radar tells Colonel Potter that he decided to go home after all it cuts to Hawkeye walking into the office, and just as Hawkeye opens the door we can see the top of the set.

Other mistake: When Potter enters the Swamp, his rank insignia is placed wrongly, with the feet of the eagle pointing to the front of the collar. The feet should point toward the shoulder side of the collar, as they are the next time we see Potter.

Other mistake: As the camera cuts to Hawkeye and Margaret working, the man behind her is holding a chest X-ray, but it's upside down, which would be useless as a guide.
