The Attack - S7-E18
Visible crew/equipment: Despite Elizabeth's objections Jim-Bob sells old Blue to Clarence, and in the next shot as Erin and Jason walk to the vehicle, the moving boom mic is reflected on the chrome surface. Then as Jason drives away, the reflection of location lighting is visible on the vehicle's rear window. (00:09:55)

(00:14:30)
Visible crew/equipment: While Elizabeth and Cindy are at Ike's getting decorations for the canteen, when Ike gets reacquainted with Callie and they're talking, two black tape marks on the floor where Elizabeth and Cindy are standing. (00:13:35)
The Diploma - S8-E4
Visible crew/equipment: While Mary Ellen takes over Nora's job by visiting the people of Blue Ridge's backwoods hollows, when Mary Ellen mounts the horse we can see the bulging outline of the mic transmitter at her back, such as when she leaves Grandma Floyd after giving her the roses. (00:21:30)
Visible crew/equipment: In the day room, after Jason calls Toni out on her crabbiness, he tells her that he's crazy about her and walks out. Then when Toni sits down at the piano, the shadow of the boom mic is visible on the wall. (00:29:35)
Continuity mistake: When Elizabeth walks out to the porch, she stands beside Zadok holding one apple in each hand in the wideshot, but in the medium shot both apples are in her left hand, but next shot it's back to one apple in each hand. (00:16:40)

The Conflict - S3-E1
Other mistake: While Martha Corinne is standing in front of the bathroom sink in the new house, we see her height compared to the mirrored medicine cabinet above the sink, and Martha Corinne is quite a bit shorter than the mirror's knob, even with her bonnet. When she reaches up to open the cabinet door, in the next closeup from behind her she is now taller than that knob. For this specific closeup The Actress was presumably standing on a stool, in order to have the perfect shot of her turning her head, then being surprised by her own reflection in the mirror. (00:54:00)
The Conflict - S3-E1
Visible crew/equipment: While Esther is pointing out of the features of the new house's bathroom to Martha Corinne, two tape marks are visible on the bathroom floor in front of John and Olivia's feet. The Conflict - S3-E1
Visible crew/equipment: Before Martha Corinne gets in the truck, when Mary Ellen tells Jim-Bob and Ben to quit eating all the berries she takes the bucket away, and as Jason turns around we can see the shadow of the moving boom mic on the seat at the back of the truck. The reflections of crew are visible on the cabin's window. (00:31:30)
The Conflict - S3-E1
Continuity mistake: Boone Walton's truck license plate number is T98254. When Blake and the U.S. Marshals drive up for the final standoff, Blake's truck license plate number is also T98254. When John-Boy runs up to Blake's truck to confront him, his front license plate number has changed again to T79825 (same as earlier, when Blake unloaded explosives), which is also the same plate number that was on another man's truck near the start of the episode. Finally, when the shooting starts and Blake's truck swerves to a stop, the back plate number is T37982. (01:27:55)
Continuity mistake: When John-Boy finishes the roof repair he carries the ladder and places it beside the tree, then he notices the unlocked padlock on the garage door. In the closeup, the open lock is inserted through the latch facing the right, but in the wideshot the lock is inserted through the latch the opposite way. (00:25:25)
Visible crew/equipment: When John-Boy tells Mr. Rudge that he'd like to know all about his deceased son, during this touching scene there are closeups as Mr. Rudge's hand strokes the vehicle that belonged to his son, and we can see the reflection of the boom pole and mic, at least two crew members, filming equipment, and another vehicle on the polished chrome surface. (00:43:20)
Visible crew/equipment: While Erin is in the forest with Lancelot to set him free, once the doe runs off Erin gets up then bends over, and the bulging outline of the mic transmitter is visible at her back. (00:47:40)
