Continuity mistake: Angela asks Bones what the rush is. When she does, Bones' ponytail is down her back. Camera changes angle and now it's over her shoulder without moving her head. (00:12:45)
Audio problem: After Zac has decrypted the code, Hodgins states he's got something. When he starts to say the line the camera doesn't show him. However it changes halfway through "got." Once the camera angle has changed you can see Hodgins' mouth and it's not moving despite us hearing him speak. (00:15:40)
Visible crew/equipment: When at the judge's home, Bones explains about needing the body exhumed. In the picture behind her there is a black line reflected from a boom arm. This can't be from anything in the house as the camera and people stay still but the reflection goes away. (00:22:15)
Continuity mistake: The amount of dirt on the skull Bones finds near the end changes drastically. (00:36:00)

Factual error: When Bones is doing the autopsy on the cannibal victims, she dictates her findings into a tape recorder. She refers to a 'low calibre' gun, rather than the correct 'small calibre' reference. Someone who deals with victims frequently, and who is so specific & detailed, would be well aware of the proper references for such things. (00:32:00)
Revealing mistake: When the town doctor confirms the clothing of the female victim you can see the victim is breathing despite missing a heart and being dead for a week. (00:33:10)
Visible crew/equipment: When Booth walks out of the bar, he puts his jacket on and his shirt rides up a bit. When it does, you can see a mic wire under his shirt. (00:36:45)
Factual error: This episode was supposed to be staged outside of a small Washington State town. About midway, they cut to a mountain background scene that is Half Dome in Yosemite National Park, California, then pan to Booth and Brennan talking to the sheriff. Anyone who does not realize an iconic landmark like Half Dome is not in Washington was asleep in Geography class.
Revealing mistake: When taking Bones to infested trees, Ranger Rivers stands sideways. In this view, it is shown that he is not wearing the handcuffs on his wrists. Instead, they are just held in his fingertips.
Continuity mistake: At the very start Bones is holding the microphone and its pointing towards her. Next shot, Dr Goodman has the microphone pointing at him. (00:00:40)
Answer: Answering my own question here but, having contacted various people involved in the production of Bones including Kathy Reichs, Hart Hanson, Eric Milligan, and Deirdre Lovegood who portrayed Taffet onscreen I finally have the answer. No-one know how she did it due to her constantly working alone. And she never revealed this information before her death.
