Continuity mistake: The assistant transplant coordinator tells Bones that another woman, Kelly DeMarco, had the same bone graft donor as Amy's. In the scene where they are analyzing the bone from Ms. DeMarco, Zach says she 'died of lung cancer 2 months ago.' Less than 10 seconds later, Booth says that she 'never smoked a cigarette in her whole life, only to die of lung cancer 8 months ago.' (00:14:50)

Continuity mistake: At 20:13 into the episode, a bunch of victims are shown on the computer screen. If you read their info, several are from Arlington, Virginia. Booth has been trying to find victims from out of state, but says nothing when these come up on the screen. Shortly after this is shown, Hodgins tells him about a victim in Lynchberg, Virginia. He then takes action as the victim is from out of state. He should've noticed the others. (00:20:10)

Continuity mistake: When Bones goes to open the vent on the wall in the mortuary while standing on the coffin, the shot from inside the grate shows her taking a step up and being exactly at head height for it. Only problem is that in the shot previous to this she was significantly short of the grate, and there was nothing on the coffin lid for her to step up onto.

Visible crew/equipment: When Booth leans down to greet Hank, his mic wire is visible against his white shirt under split in his jacket. (00:11:30)
Other mistake: Hodges and Angie find a piece of paper buried with Bones' mother and they tell Bones that the date of the movie is September 22, 1993. The ticket has two dates, September 22, 1993 and in two different places it says April 22, 1993. The Fugitive was released August 6th, 1993.

Continuity mistake: At the start we see Bones put her hair up into a ponytail from a high wide shot. Camera changes to ground level and she puts her hair up again. (00:01:10)

Revealing mistake: When Bones and Booth are driving in his SUV talking about Abraham in the Bible, you can see that Bones' side window is lowered almost all the way down, but her hair is perfectly still and not being blown around as it normally would be.
Other mistake: At the start, when Hodgins gives his findings at the scene a fly can be seen crawling over the camera lens. (00:02:00)
Continuity mistake: At the diner, before Mrs. Duncan comes to confess about murdering her husband, Brennan takes a sip of tea. Her glass at this point is less than half full. When Mrs. Duncan walks in, the same glass is now almost full.

Character mistake: In season 2, episode 4, when they find the girl buried under the cross, you can see her face, which means she was on her back, because her neck couldn't possibly bend that far around without it being noted by Dr. Brennan later in the episode (which it isn't). Later in the episode, however, Special Agent Booth tells Epps' wife that she was found buried face down. (00:14:15 - 00:17:30)

Continuity mistake: When Booth and Bones are discussing Helen Majors, Booth gets a phone call and he holds the phone to his left ear with his right hand. In the following shot, he is holding the phone in his left hand. (00:21:05)

Continuity mistake: When Angela and Hodges are working out the size of the silhouette on the wall, as Angela says "which I deny", she's got a relatively serious look on her face. We then cut to a shot from the other side of her, and she's suddenly smiling.

Continuity mistake: Booth chucks Parker's ice cream on the floor along with the napkin. When Bones picks up the napkin the ice cream has gone. (00:20:00)
Other mistake: Bones produces a gun and states that she got it herself. However it is explained in earlier episodes she is not legally allowed a weapon due to being arrested for shooting a suspect.

Continuity mistake: At the start Dwight tells Bones that the music attracts the kids. When he does, Bones has both arms outstretched. As the camera changes, she now has one arm across her body. (00:01:00)

Answer: Answering my own question here but, having contacted various people involved in the production of Bones including Kathy Reichs, Hart Hanson, Eric Milligan, and Deirdre Lovegood who portrayed Taffet onscreen I finally have the answer. No-one know how she did it due to her constantly working alone. And she never revealed this information before her death.
