Continuity mistake: A few minutes after Lee puts a bandage on Hugh's forehead, it disappears completely in one shot (as does the cut it was covering), before reappearing a moment later, but in a different position.

Under the Dome (2013)

The Red Door - S2-E9
Continuity mistake: When Big Jim communicates to the military commander at the dome, he writes on his notepad, and shows "Me and my son" to the commander; yet when he returns to his car and looks at it again, it's written quite differently. (00:24:05 - 00:24:45)

The Strain (2014)
Continuity mistake: Fet is in the sewer looking for rats and encounters a group of infected. He is holding a handkerchief to his face with his left hand to mask the smell. He has his flashlight in his right hand, and pulls out an iron bar with his left. As the camera angle changes, his left hand is again covering his mouth with the handkerchief (no iron bar), then the angle changes again, and he's holding the iron bar again. (00:35:05)

Remake A.K.A. Cloudy... With a Chance of Improvement - S8-E3
Trivia: Ralph Macchio features as a prosecuting attorney. At one point, Gus speaks a quote from "My Cousin Vinny", and the other attorney calls it "a terrific film." Macchio objects, saying "it's OK... Decent at best." Macchio featured in that movie. (00:33:05)

Haven (2010)

Haven (2010)
Continuity mistake: When the female cop tries to arrest Driscoll in the park, his Trouble kicks in and air pressure kills her. As it starts, we see blood flowing from her left ear, then it's gone, then it's there again, as the camera angle changes. (00:22:00)

Haven (2010)
Continuity mistake: When Dave and Vince arrive at the scene of the first Trouble, Dave is talking to Duke, and the camera changes from in front of Dave to just behind him. Dave wears glasses and in the front shot he has them on, but in the rear shot, they're missing. (00:09:10)

Haven (2010)
Continuity mistake: When the taxi stops to pick up the woman, it stops with its hood in front of the door of the building beside it. She gets in and finds the driver dead, and jumps out. When the police are checking it, it has moved a full car length forward. The driver put it in park when he stopped, so it didn't roll forward. (00:11:30)

Haven (2010)
Other mistake: Duke is checking old copies of the Haven Herald around the time of Lucy (27 years ago). A headline on the one he's reading is about Lucy's disappearance. A moment later, he's reading a story about a meteor shower that occurs every 27 years; but the text we can see states that it just happened, and refers to dates in 2010 (having just happened, not '27 years from now'). This paper is supposed to be a 1983 edition. Also there is fake Latin 'filler text' printed in some columns.
Suggested correction: Lucy's disappearance is at the top of the newspaper and the hunter storm is at the bottom of the same page. Also, the paper predicts the next storm to occur. Just like how we know when to expect the next lunar eclipse or the next meteor shower, so I don't see how this is a mistake.
It's not a prediction of events to come, it's an article telling about the recent event, which states it was seen on the morning of the October 21, 2010. It even compares the 2010 shower to a previous shower that occurred in 2009. The mistake is valid.

Audrey Parker's Day Off - S2-E6
Factual error: When Audrey turns on the car radio to listen to the baseball game score, the LCD display turns to the time. When she turns it off, it shows the tuner. This is the opposite of how car radios function - the sound editor must have been dubbing in the sound, and missed this).

Day Break (2006)
Continuity mistake: After running the roadblock and getting shot in the shoulder, we see Hopper's shirt with a bloody patch about 3" x 2." The camera angle changes and it is now several times larger, positioned differently and oddly-shaped (a bleeding wound would simply expand the stain on the shirt; it would not 'move'). (00:02:55)

Day Break (2006)
Continuity mistake: When the bad guys create a roadblock with their 2 cars as Hopper drives his girlfriend to her mother's house, the cars stop with one on one side of the yellow line, and the other stops with the steering wheel over the line. The camera angle changes, and now both cars are neatly, evenly positioned, with their engines sitting over the line. (00:02:05)

The Flash (1990)
Trivia: When Barry is checking the results of his tests in the police lab, he turns to the computer to view the screen. The camera changes to see the screen, and for just a moment (just a few frames) we see what was on the screen before his results - a couple of stolen car reports, one of which is that of "Joliet Jake Blues" (from the Blues Brothers movie). And yes, the car and licence are the same as in the movie. I don't know what the connection is; I assume someone on the crew loved that movie. (01:12:00)

CSI: Crime Scene Investigation (2000)
Character mistake: When Russell is reading the bio for the suspect who had dressed as a reindeer at the party and was looking for her contact lens, the bottom of the page lists "Identification Databases Checked:" and lists several states and 5 cities under each one. Alabama and Colorado have the same 5 (Alabama) cities listed. Also, the form lists several pieces of information - Class, DOB, Height, Weight, Eyes, Eyes, Hair, Restrictions, Contact Lens Serial No and Criminal History and Occupation. (Yes, "Eyes" is there twice.) The first "Eyes" field lists a name (emergency contact?), which illustrates how badly the prop department prepared this form. Lastly, despite the suspect's DOB being 04.12.1987 (either April 12 or December 4), and this episode taking place in December 2013, Russell states that she is 23 (but should be 26). (00:15:20)

Psych (2006)
Extradition: British Columbia - S4-E1
Character mistake: When Shawn is checking Ed's book of valuables, the crown that Shawn thinks Despereaux will steal, has a listed value of "950,000 - 1,000,00,000 USD." I guess it's really, really(!) valuable, enough that a whole new numbering system was made up for it. (00:30:55)

CSI: Crime Scene Investigation (2000)
Factual error: Throughout the episode, people refer to the previous murder as occurring 14 years ago. That puts it in 1999, yet we know it was in 2000, thanks to photos and other evidence.

The Terminator (1984)
Continuity mistake: In Tech Noir, as the Terminator starts firing the Uzi at Reese, a guy in a dress shirt and tie gets shot as Reese uses him for cover. He reacts to getting hit, the camera angle changes, and he reacts again. (00:35:50)
