Plot hole: When Marge is looking for inspiration for her novel, she looks at the boat painting in the TV room. On the frame, she reads it and says: "Scene from Moby Dick". Yet in "The Trouble With Trillions", Marge said that she painted it herself for Homer. So then why would she have to read the label to find out where it's from if she painted it?

Continuity mistake: When Homer goes to Ned to make sure Marge imagines nothing again, Homer puts his hand through the door, then he puts his hand through the mailbox, then Nelson puts his hand out to steal the doormat. Nelson puts his hand just over the mailbox, then in the next shot when it reveals Nelson is there, his hand is suddenly under the mailbox without enough time to move it.

Plot hole: During the Mozart skit, Lisa puts a sleeping tonic in the emperor's wine, but when she drank it she fell asleep also. How could that be if she only poisoned one glass of wine and it was the emperor's?

Factual error: Lisa, as Salieri, says she has wine for sale : "Claret, Port, Riesling ...". All three glasses on her tray are filled with red wine. Riesling is a white wine.

Factual error: On the castle where King Henry VIII is supposed to live, the flags at the top have the pattern of the English flag, being a + shaped cross on a coloured background, but it should be a red cross on a white background. Instead it's a white cross on a blue background. Those are the colours of the Scottish flag, but England and Scotland were separate countries at that point in history. And the cross on the Scottish flag is an X shape rather than a + shape anyway.

Continuity mistake: At the beginning, when Nelson is describing his family problems to the bus, the children change position between shots. Watch, for example, the boy sitting in front of Martin; in some shots he is white, in others black. (00:00:30)

Continuity mistake: Right at the beginning of the episode when all the children are on the bus we can see that in front of Nelson is Bart and behind him are Lewis, Martin and Wendall. Then when the shot changes to behind Nelson, Lewis has swapped places with Bart, Bart has disappeared and some weird kid is sitting where Lewis was. (00:00:40)

Continuity mistake: When Homer pulls up at the traffic lights, he stops over both of the lines indicating the crossing. When the woman gives him money in the next shot, he is behind the lines.

Continuity mistake: Right at the end Lisa is holding a doll head and some cards. There is a very brief shot of Bart, and then when it's back on Lisa she doesn't have anything in her hands when they hug. There wasn't time for them to be put down and you cannot see them anywhere either.

Continuity mistake: Milhouse is holding the Monopoly set in the Simpsons house. Then when his Dad comes in he isn't. Granted he could have put it down, but it isn't anywhere we can see, so he would have had to put it down behind the sofa which is somewhat unlikely.
Continuity mistake: The Simpsons' bathroom has changed position. In 'Today I am Klown' it was opposite the top of the stairs, at the back of the house; in this episode it is at The Front of the house. (00:00:35)
Continuity mistake: When Homer and Bart are fighting in the bathroom, in one shot we see the shower knock some bottles from the shelf on the wall, but then straight in the next shot they're back on the wall. (00:00:50)

Continuity mistake: Right at the very end scene, when Lisa and Maggie are sitting on the bed, you can see that there are 6 bricks on the bed. Yet when the shot changes there is only one behind Maggie, meaning two of them have just vanished. (00:20:30)
Factual error: The tongue Lisa sleeps on in the museum isn't anatomically correct. It shows the bitter and sweet taste buds as being side by side on the surface of the tongue. In reality, different taste buds are not in specific places on the tongue. I wouldn't normally complain, but this is supposed to be in a museum.
Answer: I'm not sure if this is the one you're thinking of, but an episode of "Ray Bradbury Theatre," called "A Sound of Thunder," dealt with a similar matter: a group of hunters travel back in time to hunt dinosaurs, only to find things have changed when they get back because someone stepped on a butterfly.
