Audio problem: As John and Gordon are fitting the new component on Thunderbird 5, John says, "It looks like we'll be another two hours before we're back in business. In the meantime, International Rescue is non-operational." But he is voiced by David Graham, not Ray Barrett.
Audio problem: When they are trying to escape Koopa and the little caveman speaks, then Luigi answers him, he has the Princess' voice.
Audio problem: Near the end when the family have got back in the car Malcolm says something like "who would have thought a traffic jam could be so cool, we should do this again". Near the end of the sentence we can see Malcolm's mouth and it isn't moving.
Audio problem: When Prue is at the dentist, she grabs a guy and he says, "What, what did I do?" to his girlfriend, but his lips do not move.
Audio problem: Rita is dubbed over extremely poorly - her words don't match her mouth. This is because Power Rangers is adapted from the Japanese Super Sentai series, and the footage of Rita was from the Japanese series.
Audio problem: When Abbie is trying to get Pacey's signature and says, "I'm collecting signatures," the movement of her lips doesn't match the speech. (00:15:05)
Cartman Gets an Anal Probe - S1-E2
Audio problem: After the scene where Mr. Garrison says, "You can say that again, Mr. Hat," Kyle shouts, "Give me back my brother you stupid aliens" in Stan's voice.
Audio problem: In one scene, when Warrick is in the prints lab, Nick claps his hands twice, but the sound track has him clapping three times.
Gold Is Where You Leave It - S1-E19
Audio problem: After the Apaches rescue Blue, Buck signs to their leader and says, "Vaya con Dios to you too, brother." But his lips never move.
Audio problem: In the pub with Andy, when Les is talking about Graham Norton and Les says "Ooooh, Jackie Collins!", his lips have no relation to what he's saying.
Clara's Dirty Little Secret - S1-E2
Audio problem: Season 1 Episode 2: "Clara's Dirty Little Secret". When Princess Clara and her Evil Stepmother make up, the latter thanks Clara before the scene switches to the Confession Room. When she says 'Thank you', her lips do not move.
Provenance - S1-E19
Audio problem: When Sarah goes to touch Evelyn's shoulder, you hear Sam's voice telling her to stop, except his mouth isn't moving at the time the phrase is being uttered. (00:23:50)
Audio problem: In the scene where Mr. Eko dies, he starts saying "Psalm 23." He says the first line, but when it cuts to him backing away he's half way through (note: righteousness). (00:38:30)
All Americans - November 6, 1962 - S2-E14

Audio problem: When Mr. Bean is on the bus and is trying to retrieve the golf ball, he is hitting the ankles of the passengers. When he hits the ankles of the last two women closest to the camera, they go "Ow" yet their mouths don't move. (00:17:15)
The Men That Corrupted Hadleyburg - S2-E18
Audio problem: Final sound edits were incomplete on this episode when star Pete Duel committed suicide, so his dialogue is "looped" by another actor in several places. Though it's an excellent imitation, it is most noticeably not Duel's voice when Heyes and Curry are waiting for Harry in the opening scene, at the campfire with the Tapscotts, and when Harry tells them about the trial's outcome.
Audio problem: In "Silly Love Songs" during the final song You can hear Blaine singing "I can explain." but in a couple of the shots he is lip-syncing to the "I love you". Next shot is he is back to singing his solo part.
Audio problem: The house Kimble and Marcie are holed up in certainly has remarkable soundproofing. There's a 100 mph hurricane raging outside, yet the moment they close the door, it's completely quiet inside. Throughout the episode, the storm sounds are audible only faintly, and only when the hurricane is relevant to the story line. (00:09:00 - 00:20:15)
Audio problem: Right after Helga is standing behind the "ARNOLD" on the pavement, when she says "Hey Arnold", her lips move as if she's saying it slowly (this is fixed in the 4th season).
The Lost Planet of the Gods (1) - S1-E4
Audio problem: Throughout this episode, Lucifer is audibly breathing while speaking, yet he is a robot, and unlike "Data" on "Star Trek", does not mimic human traits. (00:16:40)
