Other mistake: When Engine 51 and Squad 51 first roll in during the school fire, we see the burning building has a flat roof. After Johnny and Roy have rescued the unconscious boy from the classroom, there is a closeup of two firemen standing on a slanted, tiled roof with a hose, which is from episode 3x2, "The Old Engine", just before 17's Captain Harmell falls through the warehouse roof.

Visible crew/equipment: While Dixie, Roy, and Billy are walking away from the nurses' station down the hallway, the outlines of the mic transmitters and wires are visible on the back, center of Dixie's uniform, and the back, right sides of Roy's and Billy's shirts.

Other mistake: When Johnny contacts Rampart about the elderly man who was under a fallen bookcase, the fourth button (out of five) is depressed to record the conversation on the tape machine. However, in episode 2x5, "Peace Pipe", Brackett presses the third and fifth, when Squad 51 is at the scene of the drunk driver's accident, and in 2x10, "Dinner Date", Dixie presses the second button to record, when Squad 51 is with the little girl whose arm is stuck in the pool drain.
Other mistake: At the start of shift, after Roy digs into Johnny for calling Joanne, Station 51 is dispatched to the children lost in the flooding storm drain and time out is 08:32. Then at the scene, the officer says that they got a report about two missing boys two hours earlier, and that a witness saw teenagers pull up the manhole cover just before noon, and another witnessed the two boys playing around the open manhole. So what time is it supposed to be - early in the morning or some time in the afternoon? The times just don't sync.
Visible crew/equipment: When Johnny walks into Rampart and sees the girl whose father has tetanus, he asks her about musical instruments, and when they walk away to get coffee, the outline of the lav mic transmitter is visible on the right side of Johnny's shirt.
Continuity mistake: When Roy and Johnny are with the boy who is trapped on the crane, Johnny buckles the life belt around the boy's waist, and in the following shots the belt's direction flips around to the opposite way repeatedly. Note its stitched leather and tongue buckles.
Continuity mistake: When Johnny finds the looter's leather bag with the stolen jewelry, the bag's large front flap goes from being flipped open facing Johnny to the opposite way, between shots.
Other mistake: At the end of the episode, when 51's guys are in the kitchen waiting to see the "terrific idea", in the closeup John presses the tape recorder's third white button to play it, and we hear the dispatcher's voice, but in the next shot we continue to hear the dispatcher saying, "1305 Overland Ave, apt C, cross street, Pacific..." despite the fact that we can see all the white buttons are now up, none are actually depressed.

Other mistake: When Johnny and Roy are with the man who was electrocuted in his house, after they start the IV, Johnny says that he's going to get blankets, so he walks out the door then turns left and walks off. It's funny because the squad is parked on the street, which is to the right of the door, and the only thing to the left is the garage. Then Johnny comes back, again from the left of the doorway, with a package of the yellow blanket from the squad.

Revealing mistake: After Squad 51 is dispatched to the 'electrocution in a house', while Johnny goes to the garage to turn off the power, Roy waits at the doorway to go inside to the victim. In this interior shot facing the doorway, we see what is supposed to be the brick chimney at the side of the house next door, and the top of that set is visible. Plus the fact that this chimney is not nearly as deep as the one they passed by in the exterior shot.

Visible crew/equipment: After both siblings are carried out of the burning house, when Roy kneels down beside the boy and leans forward to set down the drug box, the bulging outline of Roy's mic transmitter is visible (till the end of the scene) at the front, left side of his shirt, and note that the actor even touches it, possibly to make sure it didn't move out of place.
Visible crew/equipment: At the refinery fire, after Roy gets into the ambulance with the patient Johnny shuts the back doors, and when the ambulance backs up the reflection of a crew member with paperwork (script?) is visible in the left ambulance window.
Other mistake: After the apartment house fire, the restored old engine is trashed by a hail of bricks. Later, when Roy and Johnny are talking about who won the first prize, The Old Engine in the back looks nothing like it did when it was wrecked on the street. Also, the shot of The Old Engine that's shown here, is actually the same footage shown in season 3, "The Old Engine", with the lighting dimmed to signify nighttime, when at the end of the episode Roy and Johnny decide to keep it. Note the exact same broken pieces of windshield, tires, hubcaps, dirt, shadows, everything.
Continuity mistake: When Brackett is first on the line with the vet, there's a closeup of Early checking the goat's heartbeat and we see the baby goat has been intubated. Brackett informs the vet of the goat's heart condition, so the vet tells Brackett to mask him with halothane and intubate. Then a minute goes by before the vet says that they should have him intubated by now, and when Brackett gets back on the line he says that the animal's intubated, even though we saw the animal already intubated in the closeup right at the start.
Other mistake: When the dispatcher sends Squad 51 on the probable overdose run, the address given is 2267 West Hill, apt 6, but when Roy and Johnny are at the door, both entering and exiting, the house number above the stairs is 11044.
Character mistake: In the morning, when Roy and Johnny are by the lockers, talking about what Johnny did on his days off, Roy tells Johnny, "I came on this morning, guys on A shift told me that they had a meeting with Brackett at Rampart yesterday." It could not have been "A shift" because Roy and Johnny work the squad for A shift, with Captain Stanley and the rest of the guys. And we know that Roy and Johnny didn't switch shifts with B or C, since the rest of A shift are there with them, working the engine that day.
Welcome to Santa Rosa County - S6-E10
Visible crew/equipment: After Johnny rappels down alone, so he can inform the sheriff of the situation with the injured climbers, when he talks to the sheriff beside the squad car, the reflections of the camera/dolly/crane are visible on the chrome corner of the sheriff's car.
Continuity mistake: When Roy and Johnny are on the Coast Guard helicopter, responding to The Boat wreck with injuries off Catalina, there are a few interior shots of them that are flipped, while they're looking out the windows. The paramedic patches are on their left sleeves, but in the flipped shots the paramedic patches are on their right sleeves.
Revealing mistake: When the Bengal tiger is on the roof, just after the officer warns, "If that cat tries to leave this roof, he's dead before he hits the street," there is a closeup of the tiger and the empty landing below the billboard. A few shots later, after Les steps on the glass, there are pieces of meat scattered on the landing below the billboard, so the trained tiger will jump onto it.
I'll Fix It - S4-E2
Continuity mistake: When Station 51 is at the scene of the oil well eruption, underneath a house which was built atop an abandoned oil field, just after Johnny tells Roy that he thinks the whole foundation is slipping, it cuts to a shot of the front porch as one of the porch columns (and part of another) breaks and falls. However, when the guys finally free the trapped man and carry him out, all the columns are upright once again.

Correct, it was J. Pat O'Malley...he also played the grandfather with his grandson when their rocket exploded, and also played "Old Bill" in the episode with Ann Prentiss, where Gage saves the little girl from the burning tree house, and her mother falls in love with him.
