Other mistake: When Roy and Johnny are waiting in the staff lounge for Dr. Morton, Johnny opens the newspaper's sport's section and sees Brice's picture. In the page's close-up, two paragraphs in two unrelated articles are the same, and they also have nothing to do with the title of the articles. The first paragraph begins, "Thus at this conference all our governments..." and the second begins, "Of no less importance was the common recognition..."
Continuity mistake: After watching Brice's interview on TV, all the guys sit down at the table to eat dinner, and there is one jar of mustard on the table. When Roy smears mustard on his bun, Chet motions for the mustard with his eyes and Roy slides it over to him to use, but in the next few shots the open jar of mustard is either in front of Roy or Chet, depending on the angle of the shot. The milk container keeps switching places as well.

Visible crew/equipment: When Johnny is kneeling on the sidewalk beside the woman who fainted, the outline of the mic transmitter can be seen at his left side, above his belt.

Revealing mistake: When Johnny, Roy, Vince, and the wounded officer are hiding between the bakery truck and car, before the multiple bullet holes actually appear on the side of the truck, those areas where the holes will show up have off-color markings, prepared for the stunt.
Bottom Line - S6-E17
Visible crew/equipment: A man walks into the ER with a white phosphorus burn on his arm, and when they wheel him into the treatment room, an actor's T-mark is visible on the floor below the gurney.
Visible crew/equipment: When the guys are on the tower ride during the rescue, the shadow of the chopper (and its rotating rotor blades) doing the aerial filming becomes visible, when the ride reaches the bottom. Then the shadow of the entire chopper moves in directly in front of the ambulance, as they load the ambulance.
Visible crew/equipment: At the station, when Johnny and Roy talk about The Stewardess and then while Chet is mopping the floor, the bulging outline of the mic transmitter is visible on the front, left side of Roy's shirt, above his belt.
An Ounce of Prevention - S6-E13

Visible crew/equipment: In the ambulance, when Roy administers the bicarb and then the isoproterenol IV, the outline of the mic transmitter is visible on the left side of the third paramedic's shirt, as he's doing chest compressions.
Continuity mistake: During Johnny and Roy's flight to LAX there are a few shots of the airplane in mid-air and when it lands, and the type of airplane changes - note the engines and tail.
Visible crew/equipment: At the start, when Johnny is telling The Stewardess about the underwater rescue and in following shots, the outline of the lavalier mic wire and the dark strap (that holds the transmitter) around Johnny's waist are visible on the back of his shirt, and the mic transmitter is visible on the front, left side of his shirt, above his belt.
Visible crew/equipment: After the airplane rescue, at Rampart when Brackett congratulates Roy and Johnny on a job well done, the outline of the mic transmitter is visible on the front, left side of Roy's shirt, just above his belt.
Continuity mistake: While on the way to the parade in The Old Engine, when Johnny and Roy deal with the fire rescue and reach the roof, Johnny has a rope bundle swung across from his left shoulder to his right hip. However, in the next shot as they run across the roof, the bundle is now swung from his right shoulder to left hip, and the rope is also bundled differently.
