Continuity mistake: After John heads down to the victims of the cave-in, when they send down the drug box with the HT, the trauma case is still beside Roy, but when Johnny asks for the blankets the trauma case is gone. Then Johnny gives Roy the victims vitals, and when Johnny goes to the vent pipe to get the blankets they just sent down, suddenly the trauma case is down below with him, and it's open, but Johnny never retrieved the case before getting the blankets. Additionally, Roy tells Rampart that the victim has a "large discoloration on the chest", even though John never said anything about it to Roy, when he gave the victim's vitals over the HT.
Visible crew/equipment: When Station 51 is dispatched to the motorcycle accident, just as Engine 51 rolls out of the apparatus bay, the reflections of array lighting and two reflector screens are visible on the side of the engine.

Continuity mistake: Roy and Joanne are going to be on a TV quiz show. When the guys are eating soup and sandwiches, Captain Stanley asks Roy, "You nervous about being on the tube tonight?" However, Roy didn't do the quiz show that night, because later we see they're on their next shift, and Roy is getting into his uniform when Johnny says "Tonight's the big night," and Roy tells him that Dwyer will do the rest of his shift when he leaves early.
Visible crew/equipment: After Johnny and Chet begin their arm wrestling, when dispatch drops the tones, there's movement at the front seats of Engine 51 where the camera is located, while Captain Stanley is at the radio base station and Mike is putting on his turnout.

Visible crew/equipment: At World Picture Corp, after the rescue of the guy with the broken clavicle trapped up on the joists, Morton tells Roy to transport the patient as soon as possible. It cuts to a shot of Squad 51 following the ambulance to ER receiving, and as they drive through the underpass, it's from the POV of inside the squad. Some location lighting becomes visible on the right side, and the reflection of the cameraman's legs are visible on the windshield, as he's seated in the passenger seat. We know that one of the paramedics is in the ambulance, so there should only be a driver in the squad.

Other mistake: When Johnny and Roy take the elevator with the nurse, they take the elevator near the nurses' station, at the south end of the ER wing, and Johnny follows the nurse onto the 4th floor to try and get her name. This is completely impossible considering the actual floor plan of the hospital, since in all of the exterior shots of the ER it's only 2 storeys.
Visible crew/equipment: When Station 51 is dispatched to the traffic accident, while en route there's a shot from the POV of the top of Squad 51, and the reflection of the cameraman, who's wearing a yellow shirt, is visible on the lightbar's red dome and chrome surface.
Continuity mistake: When Dr. Early and Marcia's father stand outside her semi-private room, number 104, we see treatment 4 is right beside it. There are two problems with this bizarre situation. First, when Dixie gave Early the lab results about Marcia's possible pregnancy earlier, treatment 2 is right beside treatment 4, not the semi-private room 104. Second problem is that when Early and the parents walk into Marcia's room, in the interior shots we can see that there's a perpendicular wall right beside the doorway to its left, where the hallway ends. Other mistake: Richard is in room 603 and jumps out of the 6th floor hallway window. In the long shot of the exterior of the building, we see that he's lying on the ledge of the row of windows that are second from the top. The elevator shows 8 floors, which means that he's actually lying on the 7th floor's window ledge.
Visible crew/equipment: At the apartment complex fire, when Truck 82 raises its ladder, we can see the second floor balcony behind the rig, and there is a crew member wearing a long sleeved blue shirt and white hat, standing in front of the corner apartment, where there should be smoke pouring out, but there is none. The reflection of the cameraman/crew member wearing a white hat also becomes visible in Truck 82's wing mirror.
Continuity mistake: During the fire at the apartment complex, when John and Roy go into the apartment to rescue the woman, they throw a coat over her, but then the explosion occurs knocking John and the woman unconscious. When Roy puts her over his shoulder and starts walking out the coat is not over her, but when he gets to the balcony she's covered by the coat, yet when the Pasadena fireman goes back for John, that coat is still lying on the floor.
