Continuity mistake: Tapp is watching the Amanda tape. Jigsaw says "look" and then Tapp rewinds the tape for 2 seconds. He presses play and Jigsaw says, "Look around" He should have said the word "mate" at that point. (00:43:40)

Plot hole: Rigg plays the woman's tape without rewinding it and it starts at the start of the tape. The woman would have listened to the tape to know the knife was under the TV. But why would she rewind the tape to the beginning after? (00:27:00)

Continuity mistake: At the end of Saw VI, Hoffman has got a massive chunk of cheek missing from the reverse bear trap. However in this film, which continues from Saw VI, he has not got a chunk missing. (00:09:00)

Continuity mistake: When on the chat show, Bobby says about the trap being "Something I never anticipated". When he does, Cale's hands move instantly. In one shot they are stretched out, in another they are not. (00:13:35)

Continuity mistake: Dr. Gordon rips open his envelope near the start. When he does he rips it with the "A" in "Lawrence" still visible. Camera cuts and the rip has moved down to take off half of the "W". (00:06:45)

Continuity mistake: Spoiler: Right at the end, we find out that Saw III and Saw IV were happening simultaneously, as demonstrated by Strahm walking in and killing Jeff. We then see Hoffman close the door on Strahm, locking him in the room. Saw V continues from the exact moment when Hoffman shuts the door on Strahm. In Saw V, Strahm quickly kicks the door with his foot just seconds after Hoffman has closed it. However in Saw IV, we see Hoffman walking away. In this shot we should hear Strahm kicking on the door.

Audio problem: Hoffman sets fire to the Jigsaw evidence with a Zippo lighter. You hear him open the lighter and set fire to the petrol. He then leaves the lighter. But you hear the lighter click shut again. (01:17:40)
