Continuity mistake: In George's memorial service, the extras are totally different in the shots from George's point of view to the shots from the back of the room. Look for the woman with brunette hair and orange overalls - she's there in the forward shots but missing in the reverses. The chairs, tables, props, costumes and character placements are all completely discontinuous. (00:12:15)

Continuity mistake: When Mohinder is back in India, his mother tells him that his sister Shanti died at the age of 5, when Mohinder himself was 2. In "The Hard Part", Mohinder tells Molly Walker that Shanti died before he was born, and shows her a photo of Shanti (who looks 5 years old) and his pregnant mother.
Mr. Stenchy - S1-E4
Continuity mistake: The second time that Jumba shoots Gantu with the orange string that wraps him up, the highest it gets is his shoulders, however, when Gantu falls to the floor, the string is now in three ringlets above his forehead.
Continuity mistake: Carter and Cadet Haley enter the Stargate and we see them through the "puddle." When they come out on The Other Side, their positions are reversed.
Continuity mistake: The burning fuse of the dynamite bundle on Brisco Jr.and Lord Bowler's table changes location as it burns - some shots the flame is close to the dynamite, other shots the flame is further from the dynamite, then back to close.
Continuity mistake: When the Thasians restore Yeoman Rand on the bridge she's barefoot, but when Charlie begs Kirk not to let the Thasians take him because he can't live with them anymore, we see Rand wearing a white fluffy slipper (which she wasn't holding when she reappeared), then she's back to being barefoot again. Continuity mistake: After Brian finds the note Alf left, we see William put the note under the microscope to inspect the tear drops. When William says "I'm going to go find him," he stands up and we see him leave the note on the microscope. When the camera cuts to William walking through the door, he all of a sudden has the note in his hand that was left on the microscope. (00:12:55 - 00:13:35)
Continuity mistake: A henchman places a briefcase up-side down on a bench, opens the locks and starts to open the briefcase with the locks above the lid latches which are flipped open toward the floor. Instantly the briefcase is right-side up with the locks down and the lid latches up. (00:26:11)
Old Wounds - S1-E1
Continuity mistake: When the Krill shuttles land, they park on either side of the entrance to Epsilon station, but when the Orville crew are escaping, the Krill shuttles are gone momentarily (during the firefight), only to return to their proper places a moment later as the Orville shuttle takes off. (00:33:40)
Continuity mistake: At the end of the episode, Odo has sent everyone affected by the telepathic matrix to a cargo bay. Sisko and O'Brien remove their com badges, throwing them aside before heading to the bay. After everyone is cured, Sisko straightens back up and, in the closeup, you can see that he has a badge on. In the next closeup, it's gone again.
The Robinsons Were Here - S1-E4
Continuity mistake: Penny is holding an envelope in her hands, then turns around to follow the siblings and the envelope is gone. Just a cut later Penny has the envelope back. (00:32:30)
Episode Nine - S2-E2
Continuity mistake: The necklace James took from the coconut in Dr. Jacobi's office in the previous episode had a gold chain. When he gives it to Agent Cooper in this episode, it's on leather.
Continuity mistake: As Starling prepares to launch the timeship from the bay behind his office, there is a shot of the exterior of the ship. In the background of this shot, there is a white wall on the right. This wall carries the company name and logo. However the name is misspelled here as Chronowerks, instead of Chronowerx as was shown on the outside of the building and behind Starling and Janeway when they were in the office.
Continuity mistake: Even after KITT's voice modulator changes to the three bars in the episode "Hearts of Stone," any shot of KITT starting himself up still shows the old voice modulator (a simple red square).
Battlestar Galactica (2003 Miniseries) - Part 2 - S1-E2
Continuity mistake: In the last battle scene in the miniseries, above Ragnar, all the civilian ships Jump away and Commander Adama orders all the vipers to land inside Galactica's landing bay. At one point, Dualla reports that two vipers are still out in the fight - Starbuck and Apollo. When Starbuck pushes her viper and Apollo's towards the landing bay, there's a shot of one of the basestars firing a missile, which hits Galactica. In this scene, we can see (and hear) 3 vipers, one of which is firing at a Cylon raider in the distance. They couldn't be Starbuck or Apollo, since Starbuck's viper and Apollo's were joined together and the mystery vipers are not, nor are they any of the other pilots, since only Starbuck and Apollo's vipers were still out, as Dualla has stated.
Time Squad - S1-E4
Continuity mistake: During Blake's first encounter with Cally, the smudges on the front of his tunic (including a big chalky footprint left by her boot) disappear, reappear and rearrange themselves several times between shots. (00:31:20)
Saga of a Star World (1) - S1-E1
Continuity mistake: When Baltar turns to face the two setting suns, we see the lower one at the left has what could be a boat in its reflection on the water. The higher one on the right is over a mountainous area on the horizon. The next shot is of Baltar in close-up, then the next returns to the horizon to show us Baltar's view, which shows just ONE setting sun. The one on the left has vanished though there is room enough for it to have been seen in this shot. (00:40:15)
Continuity mistake: When he introduces himself to Millicent inside the train station, Paul is wearing a dripping wet raincoat. It stays wet in the close-ups, but in the next several inter-cut two-shots, it's completely dry. (00:11:15)
