Audio problem: When Mike is outside, in the strong winds, yelling "Jody," his mouth doesn't match up with the audio, and sometimes his mouth doesn't move at all. (01:12:00)
Audio problem: As soon as Nemo begins playing the record, we hear a woman say "Ready, professor?". However, the actual recording doesn't start until the needle is put into position, which is after she says that.
Audio problem: After Stark escapes, he is walking in the desert, and crests the brow of a sand dune. Then, helicopters burst into view over him and you hear them for the first time. Stark was on the brow of the dune and should have heard them long before.
Audio problem: In the scene at Willard Whyte's house where Thumper and Bambi are fighting Bond, Bond gets thrown into 3 cylindrical statues. Just as the shot cuts, you can see the last column beginning to topple but there is no sound as it crashes to the floor in the next shot.
Audio problem: When Mario and Luigi are in the cave, walking along the cliff, Mario shouts "Watch out" but his mouth never moves. (00:21:25)
Audio problem: When Kirk says, "Stop energizers," watch his lips closely - he really says, "Stop engines." Mike Okuda points this out in the text commentary on the Director's Edition DVD. The reason the line (and also the one following it) was redubbed was because someone pointed out during post-production that the engines couldn't simply be stopped on a dime. (00:39:08)
Audio problem: When you first see Stokely she says 'crash and burn, Casey' but if you look carefully you notice that her lips don't move with what she is saying.
Audio problem: When Tom Skerritt first "interfaces" with Mother after being awoken, he says, "Good morning, Mother" - it is not even close to being Tom Skerritt's voice. According to the book 'The Making of Alien' this line was dubbed in by Ridley Scott as Tom Skerritt wasn't available at the time.
Audio problem: In the scene where the Grabboid is pulling the truck with the chain attached and the guy is trying to shoot it, the gun noises do not match up with the muzzle flashes at all. At one point it is obvious he fires 2 shots, and yet 4 "bangs" can be heard.
Audio problem: As the trio break into the club, they take on some bouncers. One of them gets his gun turned on him by Trinity, pushed right into his chest - we hear two gunshots, with corresponding wounds, then a third shot which has no effect. (00:14:50)
Audio problem: When Mary is reading Peter Pan to Gertie, her shadow's lips don't match up with the words she is saying.
Audio problem: When the female TX is stopped by police during the intro credits, before the officer speaks you can see the TX's lips moving saying "Is there a problem officer" but her audio is muted. (00:08:50)
Audio problem: During the "Whale of a Tail" scene, there are several instances when Kirk Douglas' hands are away from the guitar strings, to ring the bell or twirl the guitar, yet the sound of the guitar strumming continues without missing a beat.

Audio problem: When John Rivers is installing his new audio equipment, he says to his son "So you can watch your favourite show and I can watch my favourite show". John's mouth movements don't match what he is saying.
Audio problem: When Judge Dread and Rob Schneider are running to escape from oncoming guards they are stopped by one guard right before they leave on the motorcycle. The guard says "I knew it was you Dredd." The camera focuses on him before he is done saying that but his mouth is not moving.
Audio problem: When Ash receives the letter from the Dragonite, we hear Togepi say "Togepi?" but its mouth doesn't move.
Audio problem: When Cole and Railly discover the twelve monkeys symbol on the walls, Cole grabs Railly's arm and shouts "Hey!" twice, but his lips don't move the second time. (00:57:45)
Audio problem: The sound for the Cessna 208 that our characters take to Isla Nublar is that of a piston engine prop plane, while the plane is actually a turboprop plane. The start-up and cruising sounds are significantly different from what it should be in real life. (00:20:00)
Audio problem: When Malone and Roxton are fighting, Roxton hits Malone's hand, but we hear the sound effect used for a punch to the face.
