The Time of Angels (1) - S5-E4
Continuity mistake: When the Doctor is ready to catch River Song, the TARDIS exterior shown is that of the Tenth Doctor's, not Eleven's. In particular, the St John's Ambulance logo is missing. (00:04:35)
The Time of Angels (1) - S5-E4
Character mistake: When Amy says "Doesn't anybody need me?" from the door of the dropship, there's a misspelling on the door: "Trip Hazzard" instead of "Trip Hazard." (00:16:19)
The Time of Angels (1) - S5-E4
Continuity mistake: When the Doctor is looking through a hole in one of the pages in the book on the Weeping Angels and River is asking him how early in his timeline this is, which of his hands is on the page changes between shots. (00:17:26)
The Time of Angels (1) - S5-E4
Continuity mistake: When River shows Amy and the Doctor the video of the Angel, next to the Doctor on the ceiling is a bar with two cloth handles, one on each end. The Doctor accidentally rips off the handle closest to him and the viewscreen. A minute later when Amy is alone and we are looking at the Angel video, this handle is back up. It's gone again in subsequent shots.
The Time of Angels (1) - S5-E4
Other mistake: At the end when the Doctor is seen shooting the gravity globe, he does not actually move his finger but the pistol still fires.
Revealing mistake: When the Doctor's jacket is being held by a Weeping Angel while he's monologuing about why they haven't killed him yet, he moves at one point and the Angel's hand moves with him, despite the fact that they're normally effectively stone in situations like this. (00:12:16)
Continuity mistake: When Amy lies down on the tree stump, her hair is all over her face, but on the wide shot her hair is behind her head. (00:12:46)
Continuity mistake: After the Doctor finds out River's in Stormcage, when he pulls out of the pocket computer to look at the readings from the time crack, it rotates in his hand between shots. (00:19:24)
Factual error: The all important date of 26/06/2010 and it ticks over on Amy's alarm clock right at the end of the episode. However it shows 11.59 AM and flicks over to midday (12.00PM) the next day. The date should've stayed on 25/6 or, as I suspect was the intention, the clock should've read 11.59 PM and then ticked over to 12.00 AM thus changing the date correctly. (00:41:00)
Continuity mistake: In the last scene, when Amy attempts to seduce the Doctor, she pulls his suspenders off his shoulders and he pulls them back on. As a result, the suspenders change position between shots.
The Vampires of Venice - S5-E6
Audio problem: When Francesco says "Did you just say something about Mummy?", his lips hardly move because of the fang prosthetic in his mouth, revealing that the line was dubbed back in afterwards. (00:39:00)
The Vampires of Venice - S5-E6
Revealing mistake: When the Doctor begins climbing a pillar on top of the bell tower, we can see white sky on the central arch. Two other arches in the very same shot show really dark CG sky. (00:42:17)
The Vampires of Venice - S5-E6
Factual error: Isabella is a black girl living in 1580 Venice. So how is it that she has perfectly straight hair, when modern hair care products won't be invented for centuries?
Amy's Choice - S5-E7
Continuity mistake: When Rory and Amy are running from the Eknodine, there is no mud on Rory's back, but he was thrown on his back into mud a minute before. (00:23:06)
Amy's Choice - S5-E7
Continuity mistake: When Amy puts on her poncho and says "If we're all gonna die, let's die looking like a Peruvian folk band", the slit changes orientation between shots as she walks to stand between the Doctor and Rory; from across her shoulders to pointing downwards. And no, she doesn't adjust the poncho onscreen to account for this. (00:27:35)
Audio problem: After landing, the Doctor comes out and jumps in place several times. As he does, the sounds of his landings don't sync up with his motions. (00:04:55)
Continuity mistake: When Amy is being pulled into the ground, there's a shot of her arms where the Doctor's hand gripping her left arm is missing. (00:13:38)
Plot hole: When the Doctor realises that something is coming up from below ground, he tells Nasreen and Tony that at the speed the three transports are coming, the underground arrivals will reach the surface in 12 minutes. The rushed preparations that ensue, including hurrying from the drill hall to the old church and setting up a surveillance network outside, couldn't have taken less than 12 minutes to set up, especially given that they also had to: inform the others trapped inside the shield (Rory, Ambrose and Elliot), inform Rory and Ambrose that their significant others (Amy and Mo) were missing, run between the old church and the block of houses and gather supplies. And, lest we forget, there's also Rory and the Doctor arguing about what happened to Amy and Ambrose's initial skepticism as to what is occurring.
Continuity mistake: Throughout this episode, Amy's earrings seem to disappear and reappear.
Continuity mistake: When Ambrose, while standing with Elliott, admits to killing Alaya, in the first shot, Elliott is standing in front and to the right of Ambrose. In the second shot, he's standing to her left. (00:27:52)
Chosen answer: 1) When Stephen Moffat took over he ignored a lot of what had been developed before (there is not in-universe answer). 2) It would have killed Rose, so the Doctor absorbed the energy. His body regenerated before the energy could do a significant amount of damage that would prevent regeneration.
