In addition, his left arm changes position between shots, between stretched out pointing away from his body on close-ups to bent with his hand resting on his thigh in wider shots. (00:08:15)

Audio problem: When the Doctor sits in the Oval Office chair, Secret Service Agents aim their revolvers at him. As they do, the sound of a pistol slide racking is heard. None of them are doing anything except raising and aiming their revolvers at him.

Other mistake: After River shoots at the astronaut going into the lake, she drops her arm. The next second, she turns around and the gun is holstered without her having made any movement to do so, and there was no sound made.
Suggested correction: The holster is low enough for the subtle movement her right arm makes to be using it. As for the sound, the removal of her gun doesn't make a sound either. The only sound heard is directly before it when she put her medical scanner away.

Visible crew/equipment: The astronaut coming out of the lake is twice in series 6: In episode 1, "The Impossible Astronaut", and in episode 13, "The Wedding of River Song." In "The Wedding of River Song", when the helmet of the suit is lifted the astronaut is revealed to be River Song. However, in "The Impossible Astronaut", when the helmet was lifted, a man's beard can be seen. Clearly the use of a double to avoid giving the reveal away too early.
Continuity mistake: When the file with the photo of Amy's arm lands in the circle, the thumb in the photo is above the 'T' in 'THIS'. A moment later, the photo's position has turned 180° so that the thumb in the photo is pointed the other way. (00:02:57)
Continuity mistake: When the orphanage director is wiping at the writing on the wall before leading Amy and Canton onward, the cloth jumps from his right hand to his left hand and back. (00:12:54)
Revealing mistake: When the girl raises her visor to reveal herself in the spacesuit, the cracks in the visor from where it was shot earlier are drawn on. (00:20:22)
Continuity mistake: After River shoots the last of the Silence, she turns her gun flat side out, and muzzle up. After the angle change, she does it again. (00:37:58)
