Continuity mistake: When Brendon is about to sacrifice the young woman he has the tip of the dagger touching her chest in one shot and in the next shot, when Prue breaks open the door, it is about six inches above her. Also, when the victim is talking to Andy she says that she felt the tip of the knife, that he was about to drive it into her, but he was stopped by three woman. (00:24:15)
Revealing mistake: Prue is needed in the storm drain to rescue Piper, who is stuck in a hole bottomed off by a metal circular guard. Prue uses her power to levitate the metal circle to her level. When the camera pans out, the metal rods used to actually levitate the metal circle are visible. (00:36:25)
Continuity mistake: After the Grimlock kidnaps the child getting the ball, Prue goes after it. But the demon opens a hole in the earth and jumps through it and it closes. Prue then reaches the spot where the Grimlock disappeared and she is surrounded by dead leaves and trees. Then it goes to another shot of Prue turning around. Suddenly the trees and dead leaves are gone and she is standing in an open area with green grass and no trees.
Continuity mistake: While Andy and Darryl are at the station talking about why Andy's been "walking around...like a zombie," Andy places a pencil behind his ear, but when it cuts to Inspector Blakely walking in behind Andy the pencil has vanished between shots, but it reappears again in the next shot. Then when Andy borrows the photograph of the knife, once again the pencil vanishes between the closeup and wideshot. (00:09:10)
Other mistake: When Andy, Phoebe and Prue are in the attic figuring out how to vanquish the ghost of Alcatraz, pages starts flipping, the book opens up to the page where the truth spell is, and Andy finds out that Prue cast that spell on him. Later when he asks how much time Prue gave him to react, she says "a minute or two". But in the episode "Truth is out there" (1-8) it was first in Prue's office when she told him, and the clock on the wall shows that it's 25 past 5. And it was 8 o'clock when Prue cast that spell. So few hours later when Andy and Prue are at Andy's door, it's almost 8 o'clock and Prue says she needs the answer now. So doesn't Andy have more like few hours rather than few minutes to react?

Continuity mistake: In this episode Leo specifically tells Prue, Piper and Phoebe not to touch the Darklighters arrow because it is poisonous however in future episodes/seasons, the Darklighter arrows are only poisonous to Whitelighters. (00:06:30)

Deja Vu All Over Again - S1-E22
Continuity mistake: At the end of the episode, Phoebe's hairstyle changes from tied back when talking to Piper in the kitchen, to down and re-done in the time from the kitchen to the front porch.
Continuity mistake: The first three episodes have zero continuity when it comes to Piper's love life. In Ep1 "Witch Trial" Piper is hooking up with the guy who is looking at the loan for P3. Then in Ep2 "Morality Bites" she is with Leo (presumably they have secretly been together for months as they discuss their many issues being able to talk when they're both so busy with magic crises and how Leo hasn't figured out how they can be together without clipping his wings). But by Ep3 "The Painted World" Piper and Dan are already together. They has been no discussion of Piper and Leo breaking up and zero mention of Piper and Dan getting together (in fact, the last time we saw Dan in Ep1, he and Phoebe were flirting pretty heavily). I am so confused and I do not at all remember these inconsistencies from when I first watched these episodes years ago. Were these shot and aired out of sequence or something?
Suggested correction: Your last statement is correct, which also invalidates your error. Ep3, "The Painted World" was produced before Ep2, "Morality Bites" but "Morality Bites" was aired first, out of sequence.
Suggested correction: Piper and Dan weren't together in The Painted World, they weren't together until she's a man, baby, a man. When Piper sees his dating tape.

Visible crew/equipment: While the Book of Shadows flips its pages the sisters enter the attic, and after Piper asks how the book does that, in the next shot the feet of a crewmember can be seen moving quickly out of shot, at the bottom left side of the screen. (00:03:15)

Continuity mistake: In 'Morality Bites', Phoebe has her premonition about being executed while watching a news report on a baseball player, right after she and the others punish Pratt. When she returns in time at the end of the episode, they all watch the news report first and only afterwards look out the window and see Pratt. (00:02:10 - 00:39:50)
Visible crew/equipment: While Phoebe begins her "fifteen minutes of channel surfing" she has the burning premonition and cries out, then in the next shot as Prue and Piper rush in, we can see the reflection of the boom mic/pole in the wall mirror. (00:02:10)
Continuity mistake: When Prue, Piper, and Phoebe return from the future, Prue and Piper hug Phoebe in relief. In the following shot, Piper wraps her arm around Phoebe again. (00:39:20)
Factual error: Phoebe is going to be burned at the stake for witchcraft like they're starting the Salem witch trials again, but witches weren't burned, they were hanged.
