The Importance of Being Phoebe - S5-E11
Revealing mistake: When Kaya throws the knife at Phoebe watch in freeze-frame and you can see that for the shot from behind her there's no knife in her hands. (00:36:10)
Continuity mistake: When Paige follows Phoebe into the kitchen and is trying to convince her that she is telling the truth, Phoebe's right earring is missing in the shots showing both women, but it is there when the shot changes to close ups of Phoebe. It is an awfully large earring to be hidden by her hair in the two-shot. (00:25:20)
Continuity mistake: Paige's headstone reports her year of birth as 1975. But in episode 4-2: "Charmed Again, Part Two," Paige spoke with the nun to whom she was given as an infant on August 2, 1977, and Paige is always described as having been "given up at birth" by Patty and Sam. (00:27:15)
Plot hole: In 'Centennial Charmed' Paige orbs to another reality where she loses all her powers. Leo tells her the this is because she was never called as a witch in this world, but Paige had most of her powers before she was called because of her whitelighter blood, so she should retain these. Leo says that Paige has assumed the life of this reality's Paige, but then adds "...or at least the life she would have lived if she hadn't been killed". If alternate Paige had lived she would have had Whitelighter powers, even if she didn't know about them, so our Paige should also have them. (00:28:00)

Continuity mistake: When Jessica approaches Paige and Glen in the church, she puts her hands on her hips. When the shot cuts to Glen and back, Jessica's arms are crossed over her chest. (00:21:30)

Continuity mistake: When Leo finds Piper on the ladder removing the "filthy" wood shingles he's wearing a crew-neck grey and blue pullover, but when Piper climbs off the ladder and Leo tells her she needs to snap out of it Leo is wearing something beige with a collar, then it's back to the crew-neck pullover. House Call - S5-E13
Continuity mistake: Leo orbed from the Manor to Hell while it was full daylight. He orbed Jessica straight to the church, there was a brief scene, and then Leo and Paige orbed directly back to the Manor, but by the time they arrived it was completely dark. (00:31:45 - 00:33:45)

Revealing mistake: When Piper made the house vanish, and she, Paige and Leo ran up to where it was, the background was obviously fake. (00:35:35)
Sand Francisco Dreamin' - S5-E14
Visible crew/equipment: After Phoebe returns with a bowl of dream dust from the remains of the Sandman, Phoebe opens and closes the door to the conservatory to view their three dream guests, then as they all turn and walk toward the sofa we can see the reflection of the moving boom pole and mic in the mirror. Also, note the door that Phoebe had closed is now wide open in Leo's closeups. (00:32:15)

Continuity mistake: In the beginning, Paige is wearing a shirt with the scrunching sleeve on the right and slanting on the left. In following scenes, the scrunching sleeve is on the left and slanting on the right. (00:07:40)

Visible crew/equipment: While Phoebe and Paige are in the kitchen with the unicorn they realise they've lost their powers, and when Leo joins the sisters in the kitchen we can see two animal handlers - one wearing a short sleeved white shirt behind the animal, and another holding the rope hiding under the table. Visible crew/equipment: While Paige and Phoebe are in the basement getting some magic from the unicorn horn, Phoebe figures out it was the Elders who sent the unicorn, and just as Paige comments that the unicorn's magic is not affected by what happens on Earth, you can see the animal handler's hand under the animal's muzzle. (00:37:35)
The Day The Magic Died - S5-E15
Plot hole: In the episode "The Day Magic Died," Phoebe and Paige first find out that magic is down when Paige tries to orb the unicorn. But, when she tries to orb it, she makes a scrunched up face that would suggest that she knew magic was out and she hoped that she could orb.
Visible crew/equipment: While Elise is telling everyone about Bay Mirror's new owner, Jason Dean, just as she warns them that he's known for shaking up companies, we can see the white T-mark and a bit of the yellow tape mark on the crew's black mat (which is gone in following shots) at the lower left of the screen. (00:09:15)
Character mistake: Paige goes undercover into the demonic market wearing a bright blue dress and blond wig, and none of the demons seem to notice she sticks out like a sore thumb.
Continuity mistake: Right after the Hawker demon attacks, Piper picks the baby up and the head of the doll is covered and you can't see the face; a quick change in camera angles shows the baby's face and the blankets are all arranged and folded to frame the baby and no time has elapsed to allow this to happen. Also, the doll used as the baby double is much smaller than the baby and is very noticeable in shots that switch back and forth between the covered doll and real baby.
Plot hole: When Phoebe and Paige are be sent through Piper and Leo's past we see things that that cannot be true. 1. When Leo was dying in "Love Hurts" you see Piper and Leo orbing, but in the actual episode they walked down the stairs. 2. In "Just Harried" we see the cake fall and the Statue skid on the floor, but in the actual episode it was caught under the cake.
Continuity mistake: Whilst Paige and Phoebe are jumping through time, they witness Piper telling Leo she loves him for the first time. After she tells him, Leo orbs Piper and himself away. This doesn't make sense as the first time Leo orbs Piper is in season 2 Episode 8 - the water demon episode - which is actually further on in time from when she tells him she loves him (which is from season 1 episode 21).
Sense and Sense Ability - S5-E20
Continuity mistake: When Phoebe loses her hearing during her meeting at the newspaper, she holds her hand up to her ear while talking to Elise. In the next shot, her arm is suddenly resting on the tabletop, with no time for her to have moved it. (00:12:45)
