Gingerbread - S3-E11
Factual error: There is a short picture of a 17th century German newspaper. The actual text however not only is written in modern High German as opposed to Mid-High German, but also is full of errors (mixing female articles with male nouns is only one example). Funnily enough, Giles' incantation at the end of the episode is in correct German. (00:30:45)

Continuity mistake: In the previous episode 'Amends', Buffy was sporting a brand new short fringe. In this episode it has grown out. Completely.

Factual error: When Giles and Cordelia are driving to save the girls from getting burned at the stake and are making the potion to make the demon show his true face Giles tells Cordelia to shred the Wolfsbane. This is a huge mistake as any Witch will know (my sister pointed this out to me) that Wolfsbane is highly poisonous and should never be touched with bare hands and Cordelia didn't have gloves or anything.
Visible crew/equipment: Near the start, just after Buffy kicks the vampire onto the roundabout, check the top left corner of the screen as she says her line - boom in shot. (00:02:35)
Continuity mistake: In the library, a book in front of Buffy begins to slide off of a stack of books. Change of camera and book is back on top of stack.
Plot hole: When Buffy enters the house to rescue her mother she uses a stake to stop the front door from closing. A vampire then grabs it and the door closes. Why did Buffy use a stake knowing that there was a vampire there when she was almost killed by a vampire with her own stake earlier in the episode because Giles had been injecting her with a muscle relaxant?
Visible crew/equipment: While Buffy, Giles, and the gang are in the library fighting the demon that came out of the Hellmouth, the camera zooms toward the door that leads into the hall, where you can see Xander being chased by the dead guys. As the camera zooms in closer towards the round window in the door, you can see its reflection in the glass when the strobe light flashes. (00:36:10)
Continuity mistake: In the basement scene, with Jack and Xander, the bomb counts down from 13 to 10 seconds. Then after Xander says his line, the bomb again shows 13 seconds.
Continuity mistake: In the school near the end, one of the dead guys looks through the round window into the library and says 'wow'. In the long shot from within the library, his face isn't in the window. Back to the shot of the window, he's still there.
Continuity mistake: When Xander's being threatened, the knife moves from under his chin to the side of his neck between shots.
Revealing mistake: Near the end of the episode when the Mayor releases the vampire from his cage and lets him cut his head in half, you can tell that the Mayor's head is a still image when it fixes itself. The expression on his face doesn't change at all and looks completely unrealistic.
Continuity mistake: In 'Consequences' Buffy says to Faith 'Less that 24 hours ago you killed a man'. In 'Bad Girls', Faith kills Finch in the alley on the first night. One night later Buffy and Angel are attacking Belthazor, the night after that is the night when Buffy dreams about drowning, so the night when Buffy says this is actually about 72 hours after the murder. Answer: She has superpowers, ergo fast movement. The reason she outruns him so easily in "Touched" is that she stops trying to actually fight him and just dodges instead.
