Continuity mistake: In the scene where Matt pulls Alice into the mansion at the beginning of the movie and the SWAT team comes through the windows, the main SWAT member pulls Alice aside and asks for her report. If you watch the scene Alice has one arm at her side and one across her chest. A split second later it shows her reflected in the eye lenses on the SWAT member's mask and Alice has her arms crossed in front of her. Another split second later they show Alice again and her arms are in the original position. (00:13:35)
Other mistake: Why does Rain get out her torch and hold it with her teeth before climbing under the train? It would have been easier to leave it in her pocket until after she climbed down. We also see, when she is working, that it has a device on the end for gripping it properly with your teeth (called a Lite Bite, I believe). Yet earlier she chooses to grip it by the barrel. Try it with a mini Maglite. It's not comfortable. (00:15:50)
Continuity mistake: When Rain is under the train using her flashlight to see if she can see something the hair hanging across her face changes position between the wideshot and the close-up shot. (00:16:00)
Continuity mistake: When Rain tries the door on the train and fails, we see her face and her hair has 2 strands on one side of her face. Camera changes and now she's got 1 strand. (00:16:50)
Continuity mistake: On the train, just after Spence falls in, Alice takes off her wedding ring and sees on the inside: Property of Umbrella Corporation. However, after she finishes off Spence, she drops the ring next to him, and in that shot, the inside is clean; or at least the half we can see when it lies still. Such a long line would take up a lot of room on such a small ring in that size text. (00:17:45 - 01:19:05)
Factual error: When the special forces arrive in the Station of the hive, the computer identifies the soldier`s weapons. The name under the picture is right, but the picture always shows an MP5, even at the soldier carrying a G36 Assault rifle. (00:19:20)
Visible crew/equipment: When the camera moves in on the security door at the Hive, the reflection of the camera operator can be seen moving in the chrome. (00:19:20)
Revealing mistake: As the team gets off the subway and heads for the door into the Hive, the Red Queen's POV shows a nifty infrared display in the lower left corner that marks their locations - it obviously matches up to the ones on screen. But the dots are static, while the people are moving. (00:19:20)
Continuity mistake: When the SWAT team arrives at the hive, the open an elevator shaft and one of them lights a flare with 2 hands. As the camera changes, he is now holding it at arm's length and with one hand without enough time to do so. (00:23:05)
Continuity mistake: Alice asks if Spence remembers anything from before it all kicked off. When she does, her hair is tucked behind her left ear. When the shot changes, however, it's no longer tucked neatly. (00:25:20)
Audio problem: In Dining Room B, the team leader goes to look at the map and states, "Maybe you're reading it wrong." However, his mouth, which is visible for the word "wrong," doesn't move. (00:27:10)
Continuity mistake: When Rain and JD are tasked with watching the prisoner in Dining Room B, Rain's hair on her right-hand side drops down and curls to the front. The camera changes and now it's curling to the back. (00:27:30)
Continuity mistake: When we see a CGI map of The Hive right before the Laser Room, the 3D thermal camera reveals a very rigidly stood up human body looking through the door's small window. When the camera cuts and we actually see Alice look through the window, she's slightly bent down so her head is on window level since she's taller. It's quite possible the CGI team just didn't spend more time than necessary to move the thermal human model's limbs and kept it in an almost default T-stance position. (00:29:30)
Continuity mistake: In the laser corridor, the team leader is stood upright when the lights comes on, but as the camera changes to a front view, he is crouched. (00:30:10)
Suggested correction: It's easy to miss, but there is a quick insert-shot in-between these two shots that shows the team leader crouching down suddenly. (The camera is focused on the back of his head as he ducks down suddenly.) It can be seen in this video (https://www.youtube.com/watch?v=rOLe15-5Xhw&t=65s) around the 1-minute mark.
Continuity mistake: When they are entering the entrance to the Queen's chamber, the Medic is carrying a bag as you can see when the door closes. When it cuts from inside the entrance the bag has disappeared. (00:30:50)
Revealing mistake: When the team leader is opening the door to the Red Queen chamber, he clips an electronic device to the door to hack the code. The device is actually a blood glucose monitor as used by diabetics - an Accu-Chek, I think. (00:31:05)
Other mistake: During the scene with the laser, the laser moves along the floor where the EMP device is located, several times. However, when Kaplan and Alice enter the room, the bag with the EMP unit is still intact. (00:31:15)
Factual error: Medic gets killed by the laser which cuts her head off, In doing this, her spinal cord is severed meaning her muscles would not be controlled. Yet she still manages to stay standing. (00:31:20)
Revealing mistake: When Kaplan is trying to turn off the lasers, take note of his keyboard. His fingers aren't touching the keys. (00:31:20)
Audio problem: In one of the scenes right before the solider get killed by the lasers and the alarm is sounding, the commander says to everyone "Hold your positions, everyone stay calm" but his lips aren't moving. (00:31:20)
