Continuity mistake: In episode 3-13: When Jack is asking for his friends to be released, Ben says ok and Jack walks off. The camera cuts to the women, then back to Ben, and Jack is by him again having walked off a few seconds earlier.
Tricia Tanaka is Dead - S3-E10
Continuity mistake: When Sawyer helps push the minivan, there are small holes on his tee shirt's collar and a large hole on the seam of his left sleeve. When he walks back into camp with the beer, the large hole is now on his right sleeve, and the left is intact. Neither are flipped shots. (00:22:30 - 00:37:30)
Visible crew/equipment: When Jack, Michael, Hurley and Charlie play their first game of golf together, another survivor named Sullivan finds them. In Sullivan's close-ups, the clear reflection of the reflector screen is visible in his eyeglass lenses. (00:22:20)

Continuity mistake: When Michael searches for Vincent after it suddenly stops raining, his shirt's large wet stain (from the rain) changes repeatedly - such as being dry where it was just wet, up to the moment when he encounters Sun.

Other mistake: Ethan kills one of the survivors. Sawyer thinks it's Steve but Hurley corrects him and tells him that it was actually Scott. They make a point of Scott being the deceased and Steve being the one people mix him up with. According to the actors playing Scott and Steve it was in fact Steve who got killed but the producers messed it up.
Exodus (2) - S1-E24
Continuity mistake: When Sayid and Charlie find Danielle and Aaron, the baby is wrapped in Danielle's sweater. The sweater jumps on and off Aaron's face, etc., depending on various camera angles. (01:07:30)

Continuity mistake: When Sun brings the luggage to Jin her face is mildly dirty. After Jin says, "Go wash up," in the next shot facing Jin, Sun's entire face is filthy, yet when she turns it's just a bit dirty again. (00:15:25)
Factual error: The location of the working dock in Sydney where Sawyer buys a gun doesn't really exist next to the Opera House, as pictured. The reality is the site has a glass cruise ship terminal, not a shabby wooden workshop.
Live Together, Die Alone, Part 2 - S2-E24
Continuity mistake: Right at the start of the final episode in season two, Sayid is walking with Jack and he is holding a torch in his right hand. The camera angle changes and the torch has jumped into his left hand.
Whatever the Case May Be - S1-E12
Continuity mistake: After Kate and Jack dig up the marshal's body, Kate opens his wallet. In this close-up, as the pocket for bills is at the top, the vertical slit with his ID is on the right and the horizontal slits with laminated cards are on the left. However, in the close-up when Jack holds it, as the pocket for bills is again at the top, this time the vertical slit with his ID is on the left and the horizontal slits are on the right. Surprisingly, neither are flipped shots. Two different wallets were used in each close-up, though similar in style, the storage slits are on opposite sides. (00:28:05)
Exodus (2) - S1-E24
Revealing mistake: When Shannon carries and drags the luggage (some of Boone's) she breaks down and Sayid speaks to her. When he says, "It won't be too much if I carry it for you," he strokes her cheek with his left hand - note the watch. After he says, "I have a hike to lead," in the next wideshot he's still holding her face with his "left" hand. But, note the lack of watch, and her large suitcase is now backwards. They flipped the shot so his right hand would appear to be his left, to keep hand continuity, but it doesn't work. (00:17:10)
Continuity mistake: When Charlie speaks to Liam outside the church, Charlie's long scarf has a mind of its own, while round his neck, as it goes from behind his shoulder to in front, and so on. (00:10:20)
Continuity mistake: After telling Danielle what is written on the back of Nadia's photo, Sayid steps closer to Danielle. When Sayid says, "The only way off this place..." in the shot facing him Danielle holds the gun up high and points it straight at Sayid's chest. Yet, in both the previous and following shots facing Danielle, the gun is pointing down towards Sayid's legs. She then lowers the gun and sits down.
Factual error: When Michael and Tom end their phone conversation, we hear a dial tone. But cell phones do not emit a dial tone when a call is disconnected.
Tabula Rasa - S1-E3
Continuity mistake: When Marshal Mars pulls behind Ray's pickup, as the third vehicle beeps, the camera pans to the back of the cab. The cab window, behind Kate and Ray, is slid open to allow a clear view of the marshal pulling behind. In previous and following shots, the windows behind them are closed. (00:30:15)
Collision (a.k.a.Old Habits) - S2-E8
Continuity mistake: In the scene where Sun surprisingly sees Michael running out from the jungle, Sun turns and faces Michael, who is on her right. In the close-up shot followed Sun is found facing to her left. It then changes back to normal in the next shot.
Factual error: The version of Brixton underground station is very lazily reproduced: the 'U' and 'D' of the 'UNDERGROUND' roundel have been the same size as the rest of the word since before Brixton station was built; the white circle is not part of the roundel; the station appears to be accessed through a plate-glass window; there is no reason for a black cab to be parked at the side of the road; also, the cars are all parked along the right-hand side of the road, which they would never be in the UK. Born to Run - S1-E22
Continuity mistake: When Tom removes the toy airplane from the lunchbox, the three propeller pieces are completely broken off of the left wing engine (all are intact on its right wing). Yet, earlier on the island, when Kate twirls the airplane in the air, before Charlie joins her, the left wing engine has grown one piece of propeller that is unbroken. There are other moments such as this, in other episodes. (00:02:40 - 00:23:50)
Chosen answer: None of the main characters died in the plane crash. Many died on the island after the crash (Jack, Charlie, Sun, Jin, Daniel, Juliet, others), some lived on the island for an apparently long time after the crash (Hurley, Ben, Bernard, Rose) but some lived lives off the island after the crash (Sawyer, Kate, Claire, Miles, Aplert, and Lapidus). Remember that Christian tells Jack that "time has no meaning here," (in Purgatory). When everyone meets at the church, they are at the end of their lives however long that may have been and will now "move on" together. They look like they did on the island because that is the way they best remember each other.
