Continuity mistake: When Brian and Quagmire are eating at the steak house, Quagmire is seen cutting and eating his steak. When he gets up from the table, the steak is whole.
Continuity mistake: Peter is sitting on the couch talking on the radio. You see the phone sitting on a table beside the couch. But when Peter picks up the phone to call Quagmire, the phone is right beside him, without him ever moving it from the table.
Continuity mistake: Death has the TV remote asking how old the TV is. As Lois talks, he puts it down, then it suddenly disappears altogether.

Answer: Lacey Chabert was the actress who originally voiced Meg in early episodes. She left of her own accord due to being in school and other acting work; Seth MacFarlane has stated there was no tension with her leaving, but it makes for a handy joke for the show.
