Visible crew/equipment: When Riker goes to meet Beata, they sit on the bed together drinking something. As Beata puts her glass down, a hand in the lower right-hand corner reaches for it and removes it for her.

Visible crew/equipment: When Lt. Chang tells the candidates, "It's unfair that only one candidate from Relva will attend the Academy this year," there are two tape marks visible on the floor between Chang and Mordock. (00:49:00)

Visible crew/equipment: When Will is wearing the (unforgettable) indigenous apparel for his meeting with Beata, the moving boom mic casts a shadow on the wall on the right side of the screen. (00:16:25)
Visible crew/equipment: When Riker tells Yar to open a channel to the Jarada, you can see the reflection of a boom mic in the workstation along the back wall behind him.

When The Bough Breaks - S1-E17
Visible crew/equipment: After finishing his talk with Harry, Riker continues to walk into the Turbolift. A leg from a light stand can be seen in the lower right hand corner as Riker walks into the lift. [Not visible in the remastered Blu-ray version]. (00:00:45)

Visible crew/equipment: During Data's second dream experience he imagines becoming a bird and flies through the Enterprise and beyond. As he flies past his father Soong standing at the anvil, a crew member is sitting on the floor by the double doors behind him. This has been edited out in some versions. (00:36:10)

Where No One Has Gone Before - S1-E6
Visible crew/equipment: Near the start, just as Riker says "You have all the time you need.", the engine consultant stops and turns around. As he does so, look above his head and you can see a microphone hanging down. The camera quickly adjusts so it's not visible any more. [Not visible in the remastered Blu-ray version]. (00:06:00)

Visible crew/equipment: While Picard talks to Rasmussen in his ready room, when Rasmussen says, "I can't get quite as worked up as you," it cuts to a wide shot, and there's a white tape mark (T-mark) taped to the floor under the desk. (00:32:45)

Visible crew/equipment: On Romulus, when Picard and Data are told, "Eat your soup, courtesy of a loyal establishment," the camera turns to face the duo dressed as Romulans, and an actor's T-mark is visible on the ground behind Data. (00:42:40)

Visible crew/equipment: After the intro, when Garvin tells Gia to go home, Data is standing on the stairs and the actor's green mark is somewhat visible at Data's feet. This green T-mark is clearer when Data hands Garvin the radioactive box he's carrying. Then, when Garvin walks over to the table, there's an orange T-mark on the ground. (00:04:55 - 00:07:40)

Visible crew/equipment: While Troi tells Riker about the Captain's grueling negotiations between the Gemarians and Dachlyds, when Picard joins them on the bridge, there are three visible T-marks taped to the floor. (00:01:50)
Visible crew/equipment: Data tells Maddox that he formally refuses to undergo the procedure, but he supports Maddox's intriguing ongoing work. After Maddox comments that Data is remarkable, Louvois takes note that Maddox referred to Data as "he" instead of "it," and when Louvois walks toward Picard the blue tape mark is visible on the floor where she stands. Half a Life - S4-E22
Visible crew/equipment: When Lwaxana and Deanna Troi are talking in Lwaxana's quarters (near the end of the episode), note the boom microphone you can see in the mirror as Lwaxana walks away from it the first time. Shadows from the boom microphone are also seen above Lwaxana's head on the wall. (00:40:20)
Visible crew/equipment: When Picard is in his quarters and Nella Daren stops by, you can see the boom mic at the top of the screen toward the left side. (00:11:30)

Yesterday's Enterprise - S3-E15
Visible crew/equipment: During the scan of the temporal rift at the beginning of the episode, the tip of the microphone briefly comes into view at the top of the screen above Captain Picard's head when Data says, "Nothing I have seen before." [Not visible in the remastered Blu-ray version].
Visible crew/equipment: In the opening scene when Data is portraying a character from Shakespeare's play "The Tempest" a small card can be seen attached to the back of the book he is carrying (likely containing his lines), but after increasing the amount of torch light the card is no longer there.
Visible crew/equipment: When Yar and Worf prepare to cut through the wall beneath the capital city, you can see the flash-powder cord before they shoot and the phaser beams get F/X'd in.
Chosen answer: The uniform does indeed change colour. It may be due to the need for a green screen in part, or perhaps some lighting altering our perception. That being said, it certainly looks green at the start, blue thereafter.
The show has been done in HD now and not sure if they fix this problem.
