Continuity mistake: In the scene where Anya is undressing a sick Xander in his basement, she lifts up his shirt and says 'Help me get your trousers off.' During the following conversation, in the close-up shots his shirt is lifted far up on his chest, but in the wider shots it's covering his whole stomach.
Continuity mistake: When Anya is confronting Dawn about Xander's disappearance and the demon boy leaves, he walks past Dawn and her attention is on Anya but in the next shot you see him walking away from behind and his sleeve is still in the shot as Dawn turns back to Anya.
Revealing mistake: Drusilla's reflection can be seen in the metal part of what appears to be the base of a shopping mall map just as she's getting up after the Judge has been blown up.
Seeing Red - S6-E19
Other mistake: At the Bronze, when Warren punches Xander the first time, Warren never actually punches him, and Xander starts to go up in the air before you even hear the sound of the punch.
Visible crew/equipment: Season 6, Episode 'Villains,' when Willow is chasing Warren through the forest, trees move out of her way to let her pass. When the trees behind her move back into place, in one shot you can see the rope they used to tug one of the trees down.
The Harsh Light Of Day - S4-E3
Continuity mistake: When Spike is in the area where the gem is located, he walks to the right of entry hole, where he finds the gem. Then Harmony appears from the right side of the screen, where there's obviously no entrance.
Other mistake: When Buffy and Angel escape from Spike, Dru and the Judge they jump down into the sewer, then run all of five metres before going up the ladder to escape. When they come out of the sewer they are in a very open area with the factory nowhere in sight. They have not travelled far enough for this to be the case. (00:39:15)
Visible crew/equipment: When Catherine and Amy get switched back to their original selves, Catherine takes the fire axe that Amy is holding by using her powers. The wire used to pull the fire axe towards her as Amy holds it up and says "Mom! Please!" is visible.
Visible crew/equipment: When a guy asks Dawn out to Winter Formal and she says "What?" - you can see a microphone at the top of the screen. (00:33:35)
After Life - S6-E3
Revealing mistake: When Dawn is possessed and breathing fire the camera changes and we can see the safety tube the fire is coming out of. (00:23:55)
Audio problem: At the start of the episode when Buffy and Dawn are with Joyce in the hospital room, Dawn starts eating jelly, and when Joyce starts speaking you can see in the left hand corner of the screen Dawn talking, but no words are coming out.
Other mistake: The newspaper that Xander is reading in the library has the same text over and over again and does not at all match the headlines. (00:18:05)
Revealing mistake: As Willow walks through the forest pursuing Warren, just after we first see Warren running, there's a shot of Willow walking into a bit of a clearing. Her shadow crosses a tree on the right (could be caused by the moon), but then a couple of seconds later another shadow is visible on a tree to the left - obviously an artificially lit forest. Continuity mistake: At the end of the episode when Buffy is talking to Giles about how she can't face her friends, her hair is over her ears for the entire conversation, but when she is about to walk away, her hair is suddenly tucked behind her ear and she couldn't have tucked it behind as that side of her head is still in view when the camera's on Giles.
Continuity mistake: The first night that the Gentlemen float through the college, they are on the 2nd floor one minute (all the room numbers start with 2), then they turn a corner and are suddenly outside room 118, a floor below.
Continuity mistake: When Buffy is giving her speech about being chosen, there is a shot of Andrew sitting on the couch next to Xander and Anya, but in several of the shots of the Potentials he can be seen standing on the other side of the room in front of the curtains. It's definitely him - his face isn't visible, but the clothes are the exact same.
Continuity mistake: According to Giles in 'Revelations' there are 12 cemeteries in Sunnydale, but in 'Becoming Part 1', amongst others, Buffy agrees to meet someone (Angelus in this case) in 'the cemetery'. If there are 12 in the city, how would they know which one she means? (00:35:40)
Plot hole: When Buffy talks to Xander at the end and he tells her to stay with Riley, she mentions that he is going at midnight. But earlier in the episode, Riley doesn't tell Buffy he is going at midnight, he says "Transport's leaving tonight" first, and at the end of the conversation he says "Unless you give me a reason to stay, I'm leaving tonight". Nowhere in this episode does he tell Buffy that he's leaving at midnight.
Tabula Rasa - S6-E8
Continuity mistake: When Anya is standing on the table, wide shots show her holding the spellbook in front of her, but closeups show her holding it tight to her chest. (00:30:30)
Chosen answer: "So goes the nation" seems to have been used on many occasions, with various different US states in the "As .... goes" section. Most commonly it seems to be California that's considered to lead the way, but probably most other states have appeared in the lead role at some point or another. Other things have also been used - no less a person that Pope John Paul II said "As the family goes, so goes the nation...". The origin of the quote format is unclear - in US politics it goes back into the 19th century, when it was Maine that held the title spot, but, while no definitive origin is known, it seems highly likely that it goes back considerably further than that.
