Factual error: When Ian has the gun while he's in the doughnut suit, he manages to fire it. But the fingers are so big, there is no way it could possibly fit into the trigger guard to be able to pull the trigger.

Batman: Arkham Asylum
Audio problem: When Batman types on the computer in Dr. Young's office, the clacking sounds are woefully out of synch with his hand movements, not to mention his fingers never actually touch the keyboard.

Other mistake: When you enter the caves just after Killer Croc tells you he's going rip you apart, if you try to use the grapple, the grapple will not stick - Batman will say that the place is falling apart, and you need to climb manually. You can't grapple any of the edges, yet, if you fall into a pit and press RB to "Escape" like most other parts of the game, he'll grapple up to the most recent edge, despite his previous claim.

Wanted (2008)
Plot hole: Just after the shootout in the grocery store, Wesley's dad nearly runs him over in the pet truck. Before this, it seemed as though he may have wanted to kill him, but he just hadn't gotten a clear shot. It's highly doubtful that he would have trusted Fox, his enemy, to swipe Wesley into the car literally one second before he would have been plowed by the truck. Being the best assassin in the entire Brotherhood, there's no reason to think that he could act careless at a time like this, especially when doing so could easily cost the life of his son.
