Continuity mistake: When Taggart shoots the pile of skin grafts, it explodes, covering him, Jack, and Jo in guts. There's nothing on Tag's shoulder at first, but after a couple of shots, there is now a slab of skin on his right shoulder.

Other mistake: When Preston puts the VR headset on Beiderman, he reacts in a very strange manner. Yes, it's better for the movie, but it doesn't make sense in-universe, as it would take very little effort to simply lift the headset off to remove it.

Other mistake: During the park chase, Preston swipes the backpack from the person that picks it up, and is still being chased by Quigly. He somehow manages to put the backpack on in a span of about 7 seconds. Considering he is pedaling for his life, and the fear of the men catching him, it's unreasonable to assume that he could have done such a feat so quickly.

But they've been there for a while. As soon as he stops, he gets out of the car to start complaining, and the truck driver has already been out of the car for a significant amount of time.

Other mistake: When the parents freak out about Brian being missing, the back of the camper is shown, as is the part of the camper that's missing due to the distortion sphere. But it doesn't make sense; there are some parts of of the camper that shouldn't be there, mainly the one piece above the ground, where the sphere scorched the pavement but left the camper's end right above it completely intact.

Other mistake: When the Darklighter shoots Leo in the attic on the Ghost Plane, the arrow goes all the way through him. He wasn't shown to be using anything other than a normal Darklighter Crossbow, and there wasn't any evidence that, since Leo and Piper were still alive, that the physics of the Ghost Plane are any different from the Normal Plane. Yet, in every other instance of someone getting shot with an arrow from a crossbow, it never, ever penetrates the person's body, and always get stuck.

Revealing mistake: Just after Piper and Leo are taken away, Paige orbs down to the ground and runs up to Pheobe, places her hand on her, and shows a blood-covered hand showing that Pheobe's been injured. But before it's shown, while Paige is running up to Pheobe, you can see Paige deliberately keeping her hand closed and facing away from the camera, because it's already there when she orbs down.

Plot hole: It's shown with the spell that the Halliwells leave for Chris in the attic that if something happens in the past, the future will immediately reflect that change. Chris had changed so much in the past that there should have been at least one thing different that mildly surprised him, even if he was careful to not give too much information to the sisters about the future.

Other mistake: It's reasonable to assume that the house can hide small things like posters and even shelves when projecting images on the walls. When Ben's two friends arrive for the party, one of the walls shows an image and the couch disappears, and that's just way too big of an object to simply phase away, given what we see that the house can do.
