Continuity mistake: When Daniel and Henley are walking up the staircase to the apartment, as she talks about Daniel's "anonymous posting" on her website, Henley has her bag strap on her shoulder. The shot changes to them reaching the top of the staircase, and the strap is suddenly off her shoulder and the bag is now behind held by her arm. (00:10:00)

Continuity mistake: After Merritt calls Daniel a control freak and Henley says it doesn't take a mentalist to figure it out, Daniel then says he would take it as a compliment. After he says this, the shot cuts to Henley looking at Merritt saying "Only he would take it as a compliment." In the next shot, Henley is suddenly looking at Daniel. (00:11:00)

Continuity mistake: When Henley is first looking at 45 East Evan Street, her tarot card is in her hand, pushed up against a cup of coffee. By the time she meets up with Daniel, the card is no longer pushed up against the coffee cup, but instead in one of the other fingers. She couldn't have moved the card, because her bag was in her right hand and the coffee was in her left. (00:09:30)

At this moment, Merritt is resting on a pillow with a T sign stitched onto it. Merritt is first shown to be resting on the T, but a couple of shots later, Merritt is suddenly on the other end of the pillow. (00:39:55)

Continuity mistake: When Jack picks up the wet cloth to shove into Agent Fuller's mouth, there is a tap in a sink with a cloth and the tap is pointed to the left. Then as Jack drags Fuller over to the garbage disposal in the sink, the tap is suddenly pointed forwards. And when Jack shoves Fuller's jacket sleeves into the garbage disposal, the tap is pointing to the right. (01:17:20)

Continuity mistake: There's a space issue with the scenes of the safe in the truck. They show Agent Fuller coming around on the (viewer's) left side of the safe to show the Horsemen video. However, when we first see the safe in the truck, there is absolutely no way a man of Fuller's size can fit in the gap between the safe and truck wall. (01:22:35)

Continuity mistake: When the car crashes on the bridge, it slides against the barrier on the middle of the road and flips over. As it slides, the bus on the next lane is seen to be more ahead of the car. The shot then cuts to Fuller yelling out "Oh, shit!" and it cuts back to he car flipping over. Only now, the bus is suddenly right next to the car, ready to drive past it. (01:23:50)

Continuity mistake: On the bridge, as Dylan and Alma chase Jack, a bus drives in the way of Jack's car. When Jack's car is shown again, a quick shot from the front of the bus is shown. Although the light reflects on the window and we can't see the driver, we can see, just for a moment, that there are hardly any passengers on the bus. Later on, when Thaddeus is explaining how the Horsemen faked Jack's death, Merritt is shown to have been the driver of the bus, and in the passenger seats are dummies, rather than the almost empty bus shown before. Continuity mistake: When Dylan and co are in the truck, finding out that his phone has a bug in it, Dylan tells the female agent to put the bug back in his phone. As Dylan says "They don't know that we know about the bug", Alma is looking at Dylan. Next shot, she's suddenly looking at the female agent putting the bug back in his phone. (01:14:05)

Continuity mistake: As Daniel, Merritt and Henley are in the elevator, the first shot of the elevator (exterior shot) shows Merritt standing a couple of inches away from the wall. A couple of shots later, Merritt is suddenly standing in the corner of the elevator. (01:36:30)

Continuity mistake: When the car blows up, visible to the right of the car is a tyre that fell off during the crash. The explosion is then seen going over the tyre. When the fire clears the tyre isn't there anymore, but it reappears a few shots later. (01:24:40)

Question: How did Dylan's team not see their reflections in the mirror in the apparently empty vault, and why don't said reflections show in the recording for that matter?

Answer: The mirror was inserted at an angle, so that the reflection was the ceiling and not what was in front of them. It's the same principle for how periscopes work.
