Continuity mistake: Early in the episode as she is on a fire escape looking to hide somewhere, the window she opens from the outside has two half panes of glass, but on the inside it only has one pane of glass.
Continuity mistake: Jamie and Adam are shooting at Police badges to see which one would potentially stop a bullet. When they get to the third badge, it is held up to the camera. It says 'Jamie Hyneman' on the badge. The badge is then shot, and shown again to the camera. This time the engraved writing on the badge has changed to 'Adam Savage'.
Continuity mistake: Kurt is still recovering from the beating he took in the previous episode with bad cuts on his forehead, checks and temple area. They are red and impossible to miss while he is in the police station. The cuts have miraculously disappeared in the next scene when he goes to visit Mona.

Continuity mistake: Mason takes the medallion back from the matriarch as he leaves her home. A little later the old lady shows up at Della's and shows the medallion to Della.

Continuity mistake: Sami takes Maya directly from the location where Tommy Dark's body is found to his home, but Maya's outfit is completely different when they arrive.

Continuity mistake: The piece of wood the detectives throw at the window has a straight Y shape. In shots of the wood on the ground, the Y is much more outstretched compared to other shots. (00:34:50)
Continuity mistake: Matlock is inside a jail cell, speaking to a man who has been accused of murder. In an outdoor scene, someone throws a raw egg at the window as Matlock looks out. Inside the cell, as Matlock turns away from the window, the splattered egg has vanished.
Continuity mistake: When Buzz and Provenza race out of the squad room to follow Rusty in the frantic search for Stroh, Buzz is in shirt sleeves with no tie. However when they arrive at their destination Buzz is wearing a coat and tie.

Continuity mistake: When the pawnbroker closes his metal gate, he's facing the wire mesh. When the shot cuts, he's instantly facing away from it. (00:10:15)
Continuity mistake: The pothole with the mud that Rupert Thorne's henchman kicks at Harvey Dent appears from nowhere. When the henchman is shown kicking the mud, the pothole is located between the police officer behind the henchman's right shoulder and Harvey's assistant Carlos on the henchman's left. However, in the previous shot, the ground shown between the police officer and Carlos contains no pothole. (00:04:35)

Continuity mistake: DI Chandler calls a suspect "Daniel Wilkie." However when the interview is done he goes and talks to Ed, saying "you've just arrested Ben Wilkie."
Continuity mistake: Near the start after the hostage situation a white van is seen stopping then pulling away. As it pulls away the crew is reflected. (00:09:15)
Continuity mistake: In the final scene the camera switches between shots from the front and from the back repeatedly. Parker's arms are folded when shot from the back, and by her sides from the front (and vice versa).
Continuity mistake: During his questioning inside the house, Clemens' hair goes back and forth from falling over his forehead to brushed back several times between takes. (00:05:00)
Continuity mistake: In the pilot episode, at the end of the car chase, officer Tom Hanson's weapon changes from a Police Special (.38 or .45) to a .357 Magnum.
Steve Austin, Fugitive - S2-E22
Continuity mistake: The background behind Hopper while he is in the man lift changes from shot to shot. In the close ups of him in the man lift's basket, there is a striped wooden wall, but the long shot of the man lift in the raised position shows an open wooden railing behind it.
Continuity mistake: Peter and Roman dig up Lisa's corpse to steal internal organs. When they cut her open blood and fluids gush from the wound. A vehicle approaches, forcing the two to flee the scene. The Sheriff and Dr. Chasseur, who arrived in the vehicle, view Lisa's corpse a few seconds later and there is no blood on or around the body.
Revelations - S1-E10
Continuity mistake: When Piper reveals herself as the killer in the dock, you can see her dropping the mask and shortly after, removing her poncho and drop it on the floor as well. In the next shot from the side of the dock where you can see Piper and Emma face to face, the mask and the poncho are no longer on the floor. (00:32:20 - 00:33:10)

Continuity mistake: When Stevie is confronted by Danny on the beach, he's wearing a gold chain with a star of David. Different angles through the scene show him not wearing it. The different takes alternate between chain and no chain.
