Continuity mistake: In the previous episode Ra's al Ghul stabs Oliver through his chest, just below his left pec. When he pulls out the sword, a wound is visible. (However, seconds later, the wound is absent - different mistake). In this episode he is shown with a wound much lower (in his lower abdomen) and also later, with a bandage around his waist instead of his chest. (00:09:30 - 00:41:30)
Continuity mistake: At the end of the previous episode Murdoch has a ring with a dark blue stone, but at the start of this episode he takes out the box and the ring has a dark red stone.

Continuity mistake: The captain of the submarine has the rank of Commander (3 solid stripes) and half-way through episode 4 the captain suddenly has the rank of Captain (4 solid stripes) as shown on his shoulder epaulettes.
Continuity mistake: When Lee tells Quincy what delicacies the Morenos have to offer on their boat she mentions 'German salami'. But in the German dubbed version and in the German subtitles she mentions "Hungarian salami". (00:20:25)

Continuity mistake: At the end, Rumpole and his wife Hilda walk home from shopping. He's carrying the cloth grocery bag in his left hand - until the shot reverses angles, when it suddenly jumps to his right hand instead. (00:48:15)
Too Many Christmas Trees - S4-E13
Continuity mistake: When Steed and Emma are driving to the estate, the car's windscreen keeps disappearing. It's there in full shots, gone in close-ups, and reappears again every time we cut back to a long shot. (00:10:30)
Continuity mistake: When Dana and Johnny are drinking wine, the amount in the glasses change between shots. Also whenever they pretend to drink, if you watch closely the liquid in the glasses doesn't even so much as touch their lips when they drink.
Continuity mistake: When Bug speaks with the beautiful widow the second time, about performing a noninvasive autopsy on her husband, her long hair continually changes position in the shots that face her.
Continuity mistake: Series 1, Episode 9: Primal. As Carter and Taggart are crawling through the access tunnel, the torch attached to the side of Taggart's goggles is pointing downwards, below his line of vision as established by the lenses on the front. As the shot changes to a close-up on Taggart, the torch is now pointing forwards, aligned with the lenses. (00:13:50)
Continuity mistake: 1.08 (Depraved Heart) Lightman is working late and his daughter drops by to bring him Thai food. As she's walking towards her father, her arms are crossed. Cut to the immediate shot from behind, and her arms are down at her sides.
Continuity mistake: Poirot is holding up a letter in an envelope. He turns it to show the front to Lady Mayfield. The flap of the envelope appears to be tucked in. The shot cuts to a close-up, but now the back of the envelope is facing Lady Mayfield and the flap is sealed. He then turns the envelope to show her the front. (00:44:20)

Tomorrow's So Far Away - S1-E3
Continuity mistake: At the end when Hannah is sitting talking to Caroline's headstone, when the shots change from filming her from the right to the left, the wind is different blowing her hair in one and not the other.
Continuity mistake: During the scene where Ned and Chuck are making a pie, Ned picks up the unfinished pie crust off the counter so Chuck can put more flour down. After she sifts the large pile of flour on to the counter, Ned moves to put the crust back down and now it is easily twice the size it was when he picked it up. There is no way for him to have rolled the crust larger off camera because Chuck was in the way and that was the whole reason he picked the crust up.
Continuity mistake: When Red Buckley is about to enter Emil Duval's house at night before discovering the black pearl, the lights inside the house are on, but when he finally enters the house they are off. Later when he goes out, they are on again.
Continuity mistake: The packages Drake and Kathy place on the table beside their coats have disappeared when they return for the coats a few shots later. (00:19:00 - 00:21:20)
